MR Number: 2-96-0050 Saint Lucie 1 Date: 06/05/96 Florida Dockets: 50-335 PWR/CE Subject: SAINT LUCIE 1 OUTAGE EXTENSION DUE TO EXPANDED STEAM GENERATOR INSPECTIONS Discussion: St Lucie unit 1 was originally scheduled for return to service on 6/9/96. The current scheduled return to service is 7/19/96. The outage extension is primarily due to the increased scope of Steam Generator (SG) inspections due primarily to the difficulty in characterizing SG tube flaws and by observed crack-like indications in the free-span area of both SG's tubes. The increased inspections will include performing a 100% Magnetic Rotating Pancake Coil (MRPC) inspection of non-plugged tubes in the hot legs, from the center vertical support to the second support plate. Ten free-span indications have been identified to date. Additionally, Florida Power and Light currently plans to significantly reduce the St. Lucie contract labor force (currently at approximately 976) to allow plant staff to complete outage work, given the increased outage time available. MR Number: 2-96-0051 Harris 1 Date: 06/05/96 Raleigh,North Carolina Dockets: 50-400 PWR/W-3-LP Subject: HARRIS PLANT MANAGEMENT CHANGES Discussion: On June 4, 1996, Carolina Power and Light Company announced several management changes in the Operations and Engineering organizations. Mr. A. Williams, currently the Operations Manager, will begin a two-year assignment with INPO. Mr. S. Sewell, will replace Mr. Williams as the Operations Manager, Harris Plant. Key changes in the Engineering organization include the assignments of W. Peavyhouse as the Superintendent-Design Control (replacing Mr. Sewell) and J. Westmoreland as Supervisor-Reactor Systems. MR Number: 3-96-0060 Hematite Fuel Fabrication Plant Date: 06/04/96 Hematite,Missouri VIA TELEPHONE CALL TO REGION III Dockets: 07000036 Subject: MINOR FIRE IN PELLETIZING PLANT Discussion: On June 4, 1996, the licensee notified Region III of a non- reportable event involving a small fire in a pre-filter. The pre-filter is located in a press-fines vacuum collection hood, located on the first floor of the licensee's UO2 pelletizing building (Building 254). The fire apparently started as a result of sparks generated from a cutting torch used by an operator to cut a hole in the hood to install a light. The fire activated a smoke detector in the ventilation system which activated the plant's emergency alarm. Emergency responders noticed a small flame in the pre-filter, manually removed it, and extinguished the burning filter with a portable carbon dioxide (CO2) fire extinguisher. The hood had recently been cleaned and contained no special nuclear material, but a minor amount of contamination was present on the pre-filter. No injuries, releases, or intakes occurred as a result of the event. Inspection of the HEPA filter bank for the hood exhaust indicated the bank had not been damaged. The licensee also indicated that the operation had been conducted without a hot work permit as required by procedure. Regional Action: NMSS and the State of Missouri have been notified.
