MR Number: 1-96-0074 Ginna 1 Date: 08/05/96 Ontario,New York SRI PC Dockets: 50-244 PWR/W-2-LP Discussion: Region I inspectors performed a GL 89-10 inspection at the Ginna Station, during the week of July 29 - August 2, and identified a concern that the two residual heat removal system core deluge valves had insufficient safety margin and thrust capability to open under design basis differential pressure conditions. Apparently, the valves also could not overcome a postulated pressure locking condition. Both valves are required to open for low head safety injection following a medium size (4-6 inch) reactor coolant system LOCA. Following several discussions with NRR and Region I management, the licensee elected to shutdown the plant to upgrade the valve actuators. The plant shut down was initiated at 8:00 p.m. on August 2, 1996 and the plant entered MODE 4 the following morning. The actuators were subsequently modified by installing larger capacity motors and power cables, and by replacing the actuator internal gearing to achieve a higher torque/thrust output. Both actuators had been upgraded and satisfactorily diagnostic tested as of this morning. The licensee is currently assessing valve operability by reviewing all engineering and modification work completed, is anticipating entry into Mode 3 today, and expects to be back online by early tomorrow morning. MR Number: 3-96-0083 Point Beach 1 2 Date: 07/31/96 Two Rivers,Wisconsin SIR Dockets: 50-266,50-301 PWR/W-2-LP,PWR/W-2-LP Subject: INATTENTION TO CONTROL ROOM INDICATIONS Discussion: During a routine control room observation, the SRI observed that no licensed operator was stationed at the Unit 1 controls. In addition, the inspector observed that the Duty Shift Supervisor (DSS) was talking on the telephone and that the Duty Operating Supervisor (DOS) was doing paper work. Neither SRO was directly monitoring Unit 1. The inspector questioned the DOS on the absence of the operator. The DOS informed the inspector that the operator had notified the DOS that he was leaving his station to get a cup of coffee. The DOS further stated that he had monitored the panels from his desk on several occasions in the absence of the operator. When questioned, both the DSS and DOS indicated that this practice was allowed by their "Conduct of Operations" procedure. The Unit 1 operator returned to his post a few minutes after the SRI entered the control room. Regional Action: The SRI determined that the observed actions were not in accordance with station procedures. The DOS should have received a turnover from the operator and monitored the panels from the operators normal work station. Both the SRI and regional management discussed this event with the Plant Manager who agreed that the practice observed by the inspector was unacceptable. In response to this issue and a similar issue identified by NRC inspectors on July 15, 1996, regarding the watching of training videos in the control room, Region III has implemented enhanced control room inspections to monitor the conduct of operations in the control room. MANAGEMENT CHANGE Discussion: Entergy Operations, Inc. announced that Dan Keuter, General Manager, Plant Operations (GMPO) at Waterford 3 was reassigned as a team member for implementation of the Memorandum of Understanding with the New York Power Authority. The new GMPO for Waterford 3 will be Chuck Dugger, who is currently the Manager, Outages and former Manager, Plant Operations, for Grand Gulf. These changes are effective August 12, 1996.
