MR Number: 2-96-0074 Harris 1 Date: 09/05/96 Raleigh,North Carolina Dockets: 50-400 PWR/W-3-LP Subject: FOLLOWUP TO MANUAL REACTOR TRIP FROM 100% POWER DUE TO LOSS OF TURBINE BUILDING COOLING (NORMAL SERVICE WATER SYSTEM) Reportable Event Number: 30954 Discussion: On September 3, 1996 at 11:42 p.m. the plant was operating at 100 percent power when flow and pressure for the B normal service water pump (turbine building cooling water) was lost for unknown reasons. Control Room operators manually started the standby A normal service water pump but it tripped after 2 to 3 minutes for unknown reasons. This resulted in a loss of cooling water to the main feedwater and condensate pumps and vacuum pumps. In anticipation of the loss of Balance of Plant equipment, control room operators manually tripped the reactor from 100 percent power. All control rods inserted completely. The auxiliary feedwater system auto started as expected on low low steam generator water level setpoint due to shrink from the reactor trip. Steam is being dumped to the atmosphere via the steam generator power operated relief valves. The plant is stable in Mode 3 (Hot Standby). Both trains of Emergency Service Water are operable to supply emergency needs. The licensee's initial review shows that the B normal service water pump suffered a impeller failure while the A pump tripped on electrical starting interlocks. Investigation continues and the plant plans to remain shutdown until hurricane Fran has passed.
