The licensee received 118 seeds on December 27, 1995, from its source supplier (Amersham) and used 93 seeds for a permanent prostate implant on January 2, 1996. The 93 implanted seeds were verified with x-rays after surgery on January 2, 1996. Following surgery an inventory of the remaining (unused) seeds was performed and the loss of 2 seeds was identified. The licensee's RSO performed direct radiation surveys of the surgical suite and linens and did not locate the missing seeds. The licensee assumes that the seeds were disposed of in its hazardous biological waste container. Regional Action: The inspection staff will review the licensee's 30 day report, required by 10 CFR 20.2201, and determine appropriate followup actions. The State of Wisconsin and NMSS were notified.
