This was the same individual who had previously reported to Millstone with contaminated hands on May 1, 1996 (MR No. 1-96-0042). Subsequent investigation revealed that the individual was also found to be wearing clothing contaminated with cesium 137 when he was in-processing at Grand Gulf Station on May 13, 1996. During this latest incident, contaminated clothing was confiscated and no other indications of contamination were found based on surveys made in the individual's vehicle and lodging. The recurrent contamination of the individual appears to be associated with activities and operations at HOLTEC International's equipment maintenance and servicing facility in Palm Harbor, Florida. This facility is currently licensed by the State of Florida. Regional Action: NRC Region II personnel are following results of investigations into this matter by representatives of the State of Florida. Two Florida State inspections have been conducted since May 1, 1996, and have not identified any programmatic contamination control problems at the HOLTEC facility. A future survey of the subject individual's residence is currently planned by the State. MR Number: 1-96-0055 Beaver Valley 1 Date: 06/04/96 Shippingport,Pennsylvania RI PC Dockets: 50-334 PWR/W-3-LP Subject: REACTOR TRIP ROOT CAUSE ANALYSIS UPDATE Discussion: While troubleshooting the cause of the Unit 1 reactor trip that occurred on May 31, 1996 the licensee discovered an indicating light circuit, that was not on the plant diagrams, installed in parallel with a main turbine emergency trip solenoid. While being tested during a routine surveillance on May 31, this emergency trip solenoid energized, resulting in a main turbine trip and subsequent reactor trip. The licensee has postulated that this indicating light circuit could not have caused or contributed to the reactor trip that occurred on May 31. Two similar indicating light circuits, also not on the plant diagrams, were discovered in parallel with the two turbine overspeed protection solenoids. The three solenoids are located in the solid state protection system SSPS turbine trip train "B." They are not described in the UFSAR. The licensee is investigating how and why the indicating light circuits were installed and what other SSPS circuits may exist that are not documented on the plant configuration diagrams. Plant management's preliminary assessment is that these circuits would not preclude desired trips of the main turbine and, therefore, considered train B operable. In support of a documented operability determination, they are doing a 10 CFR 50.59 analysis to verify that (1) the light circuits will not prevent a valid trip from occurring, and (2) the light circuits do not affect the validity of solenoid testing. The unit is currently in Mode 1. Turbine trip protection is also provided by the redundant turbine trip auto stop trip solenoids from solid state protection system trip train "A." MR Number: 2-96-0048 Brunswick 1 2 Date: 06/04/96 Southport,North Carolina Dockets: 50-325,50-324 BWR/GE-4,BWR/GE-4 Subject: BRUNSWICK PLANT MANAGEMENT CHANGES Discussion: On June 3, 1996, Mr. W. Campbell, Site Vice President, announced the reassignment of Mr. J. Lyash, currently the Operations Manager, to the position of Manager, Brunswick Engineering Support Section, reporting to Mr. W. Habermeyer, Vice President of Nuclear Engineering. He is replacing Mr. G. Gibbs, who is leaving the company. Mr. C. Pardee, currently the Manager of Operations Support, will replace Mr. Lyash as the Manager of Operations, Brunswick Plant. Mr. R. Mullins, currently an Assistant to the Site Vice President, will replace Mr. Pardee as Manager of Operations Support. The above changes will be effective June 10, 1996. SHYMLOCK (404)331-5535 _ REGION II MORNING REPORT PAGE 4 JUNE 4, 1996 Licensee/Facility: Notification: Florida Power Corporation MR Number: 2-96-0049 Crystal River 3 Date: 06/04/96 Crystal River,Florida Dockets: 50-302 PWR/B&W-L-LP Subject: FOLLOWUP TO PLANT TRIP - TURBINE VALVE TESTING Reportable Event Number: 30571 Discussion: On May 31, 1996 Crystal River 3 tripped from 100% power during the performance of the turbine EHC trip solenoid valves testing. The reactor tripped on high RCS pressure when the turbine valves started to drift closed, reducing the secondary side cooling of the RCS. Investigation by the licensee concluded that the most probable cause of the trip was debris in the EHC system preventing the block valve from seating properly. This would allow EHC pressure to slowly bleed off. During the trip, several equipment problems were noted. Turbine bypass valve TBV 11 did not operate properly. Following the trip the licensee found that the controller for this valve was not calibrated properly and readjusted it. Two main steam safety valves opened at pressures below the design setpoints. These were found out of calibration and were recalibrated. The licensee began the unit start-up on June 2, 1996 and rolled the turbine on June 3, 1996. During the turbine start-up throttle and governor valve trip test, it was noted that throttle valve #1 hesitated approximately 10 seconds before closing. Additional testing has been performed with erratic results, ranging from no hesitation to approximately 17 seconds hesitation. The plant is currently in Mode 1, approximately 10 percent thermal power with the turbine tripped. Troubleshooting and corrective action planning for throttle valve #1 continues. OF HEALTH Subject: Cs-137 BUTTON SOURCE FOUND IN THE TRUNK OF AN IMPOUNDED VEHICLE Discussion: On June 3, 1996, at 2:30 p.m., the State of Ohio, Bureau of Radiological Health notified Region III that a 10 microcurie cesium-137 (exempt quantity) button source was rejected from a scrap yard in Kentucky on May 31, 1996. The source was inside the trunk of a crushed car. The crushed car and source were returned to the Ford Scrap Yard in Cincinnati, Ohio on May 31, 1996. On June 3, 1996, the Bureau of Radiological Health dispatched an inspector to the Ford Scrap Yard. Representatives from the State of Ohio conducted surveys at contact on the trunk and found a maximum radiation level of 4 microR/hr (2 nC/kg/hr) above background. Ownership of the crushed car was traced to the City of Cincinnati. Representatives from the City of Cincinnati retrieved the button source and placed it in storage.
