MR Number: 1-96-0087 Limerick 1 2 Date: 09/27/96 Philadelphia,Pennsylvania Voice Mail Dockets: 50-352,50-353 BWR/GE-4,BWR/GE-4 Subject: IMPROPER RELEASE OF RADIOACTIVE SOURCES Discussion: On September 27, 1996, the licensee informed NRC Region I that two radioactive strontium 90 sources (0.25 uCi each) had been improperly released from the site and disposed of, apparently at a metal melting facility. The sources were contained in large, portable airborne radioactivity monitors called PINGs (particulate, iodine, and noble gas monitors). The two PINGs, which each contained one source, were released ("excessed") to a local junk dealer in September 1994. The two sources were, however, not discovered to be missing until April 1996. Each PING (of this design) normally contained five sources but one of the five sources was left in each of the two PINGs. The PINGs were subsequently sold by the local junk dealer to a facility in Trenton, New Jersey that collects and distributes metals for melting. The sources, according to the licensee, could produce a worst case contact dose rate of about 4 rads/hr (0.04 Gy/hr). The licensee indicated that, due to the location of the sources in the PINGs, their design, and the high probability that the PINGs were melted, the sources presented no significant personnel exposure risk. The licensee indicated that its review determined that the matter was not reportable. The licensee was continuing to determine the exact disposition of the PINGs and reasons the sources were not detected missing earlier. The licensee performed a comprehensive inventory of the station's sealed radioactivity sources and concluded that all other sources were accounted for. Regional Action: The regional staff continues to follow the licensee's actions in response to this event. The states of New Jersey and Pennsylvania have been notified.
