MR Number: 3-96-0001 Byron 1 2 Date: 01/03/96 Byron,Illinois RI DISCUSSION WITH LICENSEE Dockets: 50-454,50-455 PWR/W-4-LP,PWR/W-4-LP Subject: BUS FEEDER BREAKER FAILED TO CLOSE DURING UNIT STARTUP Discussion: On December 24, 1995, during Unit 1 startup, the Unit Auxiliary Transformer (UAT) feeder breaker to bus 143 (non-safety related) failed to shut. Immediate investigation identified the breaker was not completely racked into the cubicle. The breaker was approximately 1.5 inches from the cubicle face. The UAT feeder breaker to bus 143 is a Westinghouse 4.1 kV breaker. Further investigation identified two potential root causes for the breaker not completely engaging. The main current conductors, or "baskets" were slightly out of alignment. The alignment error could have made the final travel distance more difficult or prevented the breaker from fully engaging. Additionally, the levering in device was damaged. The levering in device is used to rack the breaker into the bus. The point on the levering in device where the thrust is absorbed indicated some galling and wear. The licensee currently believes a combination of the baskets being slightly misaligned and the damaged levering in device prevented the breaker from fully engaging the cubicle.
