The NRC issued this information notice to alert addressees to a hydrogen gas ignition event that occurred during the welding of the shield lid on a spent fuel storage cask at the Point Beach Nuclear Plant. MR Number: 1-96-0053 Calvert Cliffs 1 2 Date: 06/03/96 Lusby,Maryland RI P/C Dockets: 50-317,50-318 PWR/CE,PWR/CE Subject: CONTAMINATED SEWAGE Discussion: On May 25, BGE chemistry technicians detected low-level radioactive contamination in the contents of the waste treatment holding tank (WTHT). This tank collects the concentrated waste (sludge) from sinks, on-site toilets, and other non-radioactive sources for processing and eventual disposal off-site in a local sewage treatment system. Isotopes recently detected included trace amounts of cobalt-58 and cobalt-60. BGE sampled the off-site disposal location and detected no contamination, suggesting to BGE that the contamination event was an isolated occurrence. BGE management indicated that the contaminated waste would be barreled and treated as radioactive waste. An investigation into the source of the contamination and its method of entering the waste stream is ongoing. In response to recent NRC inspection findings, the BGE chemistry department changed its monitoring practices to incorporate the use of environmental lower limits of detection (LLDs). The LLDs previously used were somewhat higher than the current environmental LLDs, but not as high as the effluent LLDs. Samples of sewage collected prior to May 25, when evaluated using the environmental LLDs, showed no detectable contamination. Regional Action: For information only. Hinnant, Site Vice President, announced the reassignment of Mr. J. Moyer, Manager, Nuclear Assessment (NAS), to the position of Maintenance Manager. The Maintenance Manager position was vacant following the announcement that Mr. B. Clark had been named as the Plant General Manager for the Harris Plant (see MR Number 2-96-0046). Mr. R. On May 14, 1996, the International Brotherhood of Electrical Workers Local 605 and Entergy agreed to a contract extension until May 31, 1995, so that contract negotiations could proceed in an orderly fashion. Pick (817)860-8270 _ REGION IV MORNING REPORT PAGE 5 JUNE 3, 1996 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-96-0052 Cooper 1 Date: 06/02/96 Brownville,Nebraska Senior Resident Inspector Dockets: 50-298 BWR/GE-4 Subject: SHUTDOWN Discussion: On June 1, 1996, at 2 a.m., the licensee began a scheduled 12-day midcycle outage to address concerns with failed fuel. Fuel leakage was originally detected in April and the leak was located and local power suppressed. However, offgas radiation levels continued to increase. Projections showed that, if leakage rates were not decreased, Technical Specification iodine release limits may be exceeded in midsummer. This would necessitate a plant shutdown when electric power demand was highest. Other than identifying and removing the leaking fuel, work to be performed during the outage includes the repair of a RCS leak in primary containment on a 1-inch feedwater drain line and performance of various surveillance tests.
