Headquarters Daily Report SEPTEMBER 03, 1996 *************************************************************************** REPORT NEGATIVE NO INPUT ATTACHED INPUT RECEIVED RECEIVED HEADQUARTERS û REGION I û REGION II û REGION III û REGION IV û PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV SEP. 03, 1996 Licensee/Facility: Notification: Omaha Public Power District MR Number: 4-96-0094 Ft Calhoun 1 Date: 09/03/96 Fort Calhoun,Nebraska Resident Inspector via Telephone Dockets: 50-285 PWR/CE Subject: OVERHEATING OF RAW WATER TEMPERATURE INDICATOR Discussion: At approximately 1:30 a.m. (CDT) on September 3, temperature indicator TIC-2885, the indicator for the raw water outlet temperature from the Raw Water/Component Cooling Water Heat Exchanger AC-1A, overheated and began smoking. The instrument is located in the main control room in Control Panel CB-1/2/3. The condition lasted for approximately 10 seconds until the indicator failed. An Instrument Bus A ground alarm sounded simultaneously with the overheating condition. The overheated instrument self extinguished upon failure and, when electricians deenergized the circuit, the instrument bus ground annunciator cleared. Troubleshooting and repair of the indicator was in progress as of 8 a.m. No other components were damaged.
