The NRC is issuing this information notice to alert addressees to a recent incident involving improper installation of power-operated relief valve (PORV) internals which rendered the PORVs inoperable, and inaccurate indications of the actuation of PORVs based upon tailpipe acoustic monitoring data during testing. Benner, NRR (301) 415-1171 _ REGION IV MORNING REPORT PAGE 2 JANUARY 2, 1996 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-96-0001 Cooper 1 Date: 01/02/96 Brownville,Nebraska SRI Dockets: 50-298 BWR/GE-4 Subject: REACTOR STARTUP FOLLOWING REFUELING OUTAGE Discussion: Cooper Nuclear Station completed its 16th refueling outage, originally scheduled for 55 days, on December 30, 1995, when the turbine output breaker was closed at 9:34 a.m. The licensee completed the outage in 77 days, 22 days past their scheduled outage time. The licensee is currently at 75 percent power and scheduled to reach 100 percent power on January 4, 1996. The outage was extended primarily due to the difficulties with implementation of a major modification to the emergency diesel generators. Other major work accomplished during the refueling outage was a 10-year inservice inspection of the reactor vessel, containment testing, 125/250 volt battery replacement, safety relief valve vacuum breaker replacement, high pressure core injection system vacuum breaker modification, and the replacement of 500 feet of the steam extraction piping.
