MR Number: 1-96-0031 Nine Mile Point 1 Date: 04/02/96 Lycoming,New York Dockets: 50-220 BWR/GE-2 Subject: Planned Outage Discussion: At 9:10 p.m. on April 1, 1996, Nine Mile Point Unit 1 initiated a plant shutdown and commenced Planned Outage 96-01. The unit was shut down to repair identified feedwater heater leaks. Other activities planned during the outage include: drywell leakage and steam leak repairs, a reactor safety valve thermocouple repair, and containment and core spray pump operation to support a torus water inventory agitation test. The outage duration is expected to be five days. During the power ascension, a five-year emergency condenser capacity test will be performed, and scram time testing will be conducted on nineteen control rods. Contact: Barry Norris (315)342-4041 Richard Conte (610)337-5183 _ REGION II MORNING REPORT PAGE 2 APRIL 2, 1996 Licensee/Facility: Notification: Agreement State Broad Licensee MR Number: 2-96-0027 Duke University Medical Center Date: 04/02/96 Durham,North Carolina License No: 032-0994-7 Subject: LOST SOURCE Discussion: North Carolina, an Agreement State, notified the NRC Duty Officer on April 1, 1996, that a 7.6 millicurie iodine-125 sealed source was reported lost at the Duke University Medical Center. The lost source had been reported to the State earlier on April 1. The source had been used as a motion point source and taped to a patient. The source is believed to have been removed with the tape on April 29, and discarded with the tape. The discarded tape normally goes into a dempsey dumpster and then is sent to a local landfill. The licensee has conducted surveys of the dumpsters and other areas of the licensee's facility and has been unable to locate the source. The State continues to monitor the licensee's actions.
