MR Number: 2-96-0038 Harris 1 Date: 05/01/96 Raleigh,North Carolina Dockets: 50-400 PWR/W-3-LP Subject: UNIT TURBINE TRIP - REACTOR TRIP DUE TO A FAILED GENERATOR OUTPUT BREAKER DISCONNECT - FOLLOWUP TO EVENT 30370 Discussion: On April 25, 1996 at 9:07 p.m., the Harris plant experienced a reactor trip following a turbine trip caused by a high resistance fault on the A phase of the generator output breaker (52-7) disconnect resulting in a failure of the disconnect. All rods fully inserted and reactor coolant pumps remained operational. Several busses lost power at the time of the trip and secondary system equipment was secured due to the loss of normal service water. A momentary loss of power was sensed on the "B" safety bus. The "B" EDG started and loaded to the bus as expected. The "A" safety bus did not loss power. The licensee found that the "A" phase disconnect switch blade had not fully rotated into the jaws of the receptacle. This resulted in a high resistance and a ground fault. The licensee replaced the disconnect and verified that the other breakers in the yard were properly aligned. The licensee determined that the "B" EDG started on a valid under voltage condition (due to relay indications). The maintenance mode indication would not have prevented the "A" EDG from starting on an emergency signal (which it did not receive). The charging pump suction transfer to the RWST was due to a low VCT level and letdown isolation.
