The U.S. Nuclear Regulatory Commission (NRC) is issuing this generic letter to (1) notify addressees about safety-significant issues that could affect con- tainment integrity and equipment operability during accident conditions, (2) request that all addressees submit certain information relative to the issues that have been identified and implement actions as appropriate to address these issues, and (3) require that all addressees submit a written response to the NRC relative to implementation of the requested actions. The State conducted an investigation into the 93 rem exposure during the week of September 23-27, 1996. Both State and Duke University continue their investigations. The film badge was worn during the period from April 5 to July 5, 1996, and was reported by the vendor to show a 93 rem skin dose and a negligible whole body dose. The individual who wore the badge had only worn it for a few days during this period. The individual did not work directly with P-32 during this period, but did observe others working with P-32. Duke personnel performed a bioassay on the individual in September and found no evidence of internal exposure from P-32. Regional Action: Region II will continue to coordinate with the State. Henson (404)331-0344 _ REGION II MORNING REPORT PAGE 3 OCTOBER 1, 1996 Licensee/Facility: Notification: Agreement State, Non-Licensee MR Number: 2-96-0092 Brenner Iron & Metal Company Date: 10/01/96 Winston-Salem,North Carolina Subject: RADIATION MONITOR ALARM AT SCRAP METAL YARD Discussion: North Carolina, an Agreement State, notified the NRC Headquarters Operations Officer on September 27, 1996, that the Brenner Iron & Metal Company had received a 10 feet long, 6 inch diameter piece of stainless steel pipe at their scrap metal yard in Winston-Salem, Norther Carolina. The pipe was part of a truckload of scrap metal received from a paper mill in North Carolina. The pipe set off the scale monitor alarm which was preset to alarm at Level 2, which corresponds to a unitless numeric range reported to be 69-70. (This is not a dose or dose rate number. Note that the Operations Center event report listed a dose rate of 69-70 rad, which was not the monitor reading). Scrap yard staff also reported that a survey with a hand held meter (type unknown) read 7500-8000 counts per second on contact with the pipe. The pipe was unloaded from the truck and both ends were taped shut with duct tape. The pipe was stored in a secure area away from personnel to await further evaluation. The scrap yard company intends to have the pipe evaluated by a contractor to identify the radionuclide and activity. The State will determine whether to conduct an on site inspection based on contractor's evaluation report. Regional Action: Region II will continue to coordinate with the State. Contact: J. MR Number: 3-96-0103 Fermi 2 Date: 09/30/96 Newport,Michigan SRI PC Dockets: 50-341 BWR/GE-4 Subject: PLANT SHUTDOWN FOR REFUELING Discussion: On September 27, 1996, the Fermi 2 plant was shutdown to commence the fifth refueling outage. Major activities planned during this expected 52- day outage include: replacement of the three main low pressure turbines; replacement of all 185 control rod position indicating probes, connectors, and cables; and modifications to the general service water system. In addition, over one thousand preventive and corrective maintenance activities are planned including emergency diesel generator and reactor core isolation cooling water pump inspections.
