MR Number: 1-95-0121 Indian Point 2 Date: 09/22/95 Buchanan,New York SRI/PC Dockets: 50-247 PWR/W-4-LP Subject: SERVICE WATER PUMP FAILURE Discussion: On September 8, 1995, the 25 service water pump (SWP) at the Indian Point 2 site experienced a shaft failure. The vendor for the pump and shaft is the Aurora Pump Company. The pump had been installed approximately 40 days prior to the failure and the post maintenance testing at that time did not reveal any abnormal vibration readings. Also, there was no indication of an imminent failure based on the routine log readings. Following removal and disassembly of the failed 25 SWP, Con Edison determined the following: 1) The pump had ingested foreign material as evidenced by impact marks at two locations on the impeller and damage to the grease connection on the outside of the shaft casing. 2) The pump had been installed for approximately 40 days prior to the failure. Normal expected pump life is about 18 months. 3) A greater than expected amount of pitting was observed on the pump shaft for the length of time in service; 150 microns deep vice an expected normal 50 microns over two years. 4) Analysis indicated that the failure point on the shaft was at a pitting site. The failure site had an existing crack which had propagated substantially through the shaft prior to the impact. Cracks were identified at the other pitting sites. 5) The fractured area had been heat treated for shaft straightening to greater than 1600 degrees F. This was the normal process performed at the mill by Earle M. Jorgensen Company. This changed the crystalline structure of the metal in a localized area, effectively creating an area of dissimilar metal and may have resulted in conditions conducive to galvanic corrosion within the shaft's metallic structure. 6) Chloride levels in the Hudson River (source of water for the SWP) were at record high levels (due to drought conditions allowing sea water migration up the river) the week the 25 SWP was installed. Con Edison believes that the excessive shaft pitting may have occurred because the shaft did not have time to develop a protective corrosion layer prior to being placed in the high chloride environment. Con Edison is evaluating the effects of the identified high temperature heat treatment of the shaft combined with high chloride levels on the 410 stainless steel SWP shafts for the other SWPs at the site. (PEPCO) announced that their respective Boards of Directors had approved a merger of the two companies. The merger will create a regional energy company with customers in Maryland and Washington, DC, that will be the ninth largest utility in the nation. C.H. Poindexter, currently Chairman of the Board and CEO of BGE, will be the CEO of the new company, which has not yet been named. E.F. Mitchell, Chairman and CEO of PEPCO, will serve as Chairman of the new company until he retires in 1998, when Poindexter will hold both jobs. The merger is subject to approval by shareholders of both companies and certain government agencies, including the NRC. The merger is expected to be complete by early 1997. The visit is sponsored by the Department of Defense Cooperative Threat Reduction Program, a part of a U. S. - Ukraine bilateral agreement to strengthen Ukranian nuclear safeguards. They are accompanied by the NRR Project Manager and Russian and Ukranian translators.
