The total number of tubes plugged was 764, and the number of tubes sleeved was 2055. The licensee plans to replace all four Unit 1 steam generators during the refueling outage scheduled to begin January 1999. Contact: LEW MILLER (708)829-9629 _ REGION III MORNING REPORT PAGE 2 DECEMBER 26, 1995 Licensee/Facility: Notification: MR Number: 3-95-0197 St. Joseph Mercy Hospital Date: 12/22/95 Ann Arbor,Michigan VIA TELECON TO HQS DO Dockets: 03017041 Subject: POTENTIALLY RADIOACTIVELY CONTAMINATED WASTE DISPOSED OF AS ORDINARY WASTE Discussion: The licensee notified the NRC that on Thursday, December 21, 1995, a bag of potentially radioactively contaminated waste, including paper plates, plastic forks, and paper toilet seat covers, was taken from a patient's room and inadvertently disposed of with ordinary waste at a BFI landfill in Salem, Michigan. The patient had been treated with 125 millicuries of iodine-131 on Monday, December 18, 1995, and items that the patient may have been in contact with were accumulated in a plastic bag. Although the actual amount of contamination on the items is unknown, the amount of contamination is believed to be of minimal radiological significance. All hospital waste is passed through a portal monitor sensitive to 20 micro-roentgen per hour. No alarms were received during the time period in questions. The landfill portal monitor, also sensitive to 20 micro- roentgen per hour, did not alarm during the time period in question. By the time the hospital staff identified that the bag may have been inadvertently disposed of as normal waste, the landfill had buried the waste from that day under 30 feet of clay. Regional Action: The State of Michigan, Division of Radiological Health, was notified of this incident. Region III will review this incident at the time of the next inspection.
