MR Number: 1-95-0120 Beaver Valley 1 2 Date: 09/22/95 Shippingport,Pennsylvania RI/PC Dockets: 50-334,50-412 PWR/W-3-LP,PWR/W-3-LP Subject: INSTRUMENT ISOLATION VALVES MISPOSITIONED Discussion: On September 20, 1995, the instrument isolation valve for Unit 2 diesel generator 2-1 fuel oil daytank level instrument LIS 205A was found shut. Its normal position is open. It is believed that the mispositioning occurred when the instrument line was used to take a fuel oil sample on September 14, 1995, by the chemistry department. Two other level instruments on the same tank were found properly aligned and would have assured proper functioning of the fuel oil transfer system. On September 21, instrument isolation valve QS-173, a Unit 1 sodium hydroxide addition pump train B discharge header pressure gauge was found open. Its normal position is shut to provide isolation from the non-seismically mounted pressure gauge. This valve had been independently verified to be in its correct position two days earlier following completion of a surveillance test. Following identification of this mispositioning, the licensee verified correct positioning for all operations safety-related valves in the Unit 1 chemical addition building. In addition, safeguards checklists were completed for both units, and no deficiencies were identified. A security officer was posted at the Unit 1 chemical addition building and accompanied anyone entering the building until the licensee successfully completed the safeguards checklists. A complete walkdown of all operations valves in the Unit 2 diesel building is currently underway. The licensee is continuing their investigation, and have obtained the Unit 1 chemical addition building and the Unit 2 diesel building access records for review.
