The licensee plans to reperform the test on September 21, 1995, to confirm the test results. In August of 1990 feedwater flow was measured using a sodium dilution tracer test which revealed indicated and actual flow were identical. Currently, the licensee is attempting to determine the cause of the increased reactor feedwater flow and is assessing the impact of operating with 102.4 percent flow. The licensee believes it is likely that license core thermal power may have been exceeded but verified that thermal limit margins were greater than 10 percent. The licensee notified the NRC as required by license Condition 2.F on September 20, 1995. The Plant Manager notified the Senior Resident Inspector of this issue at 4:45 p.m. on September 18, 1995. Date: 09/21/95 Lake Charles,Louisiana LA Rad Protection Div Notified RIV Subject: PACKAGE RECEIVED WITHOUT RADIOACTIVE LABELS OR SHIPPING PAPERS Discussion: A shipment of radioactive material was received on September 15, 1995, at Omnitron International, Inc., in Lake Charles, Louisiana, without shipping papers and without radioactive labels on the external surface of the package. The shipment was received from Seoul, Korea, and contained a spent Omnitron source wire (approximately 0.5 curie of iridium-192). Inspectors from the Louisiana Radiation Protection Division investigated and found that the source exchange package (a shielded metal can) was properly packaged with DOT Yellow II labels and tamper-proof seal intact, per the instructions sent by Omnitron. It is presumed that the metal can was then placed in a cardboard box and shipped by the Korean packagers without radioactive labels on the external surface of the box. It is unknown whether the shipping papers were missing from the beginning of the shipment or lost in transit. There was no contamination nor any excessive radiation profile found for the package. The package traveled from Korea by air to Los Angeles, California, and then via UPS to Lake Charles, Louisiana. This is a second apparent improper preparation for shipment of an Omnitron spent source wire from Korea. See PNO-IV-94-065, dated December 22, 1994. MR Number: 4-95-0116 Trojan 1 Date: 09/19/95 Ranier,Oregon PHONE Dockets: 50-344 PWR/W-4-LP Subject: COURT ACTIONS EFFECTIVELY STOP LARGE COMPONENT REMOVAL Discussion: On September 7, 1995, the licensee informed Region IV and NRR that the Supreme Court of the State of Oregon issued a stay to Portland General Electric (PGE) which halted all activities related to Trojan's Large Component Removal (LCR) plan. Trojan's LCR involved removing all four of its steam generators and its pressurizer, and barging those components to U S Ecology's disposal facility in Hanford, Washington. These activities were conducted as part of Trojan's ultimate decommissioning. Don't Waste Oregon Council (DWOC) filed its lawsuits in court on September 5, after receiving information that NRC will not appeal the decision rendered by the First Circuit Court of Appeals regarding the decommissioning at Yankee Rowe. DWOC had also filed in federal courts, but at the time the federal courts had not acted on the motion. On September 19, 1995, PGE informed NRC that the state court lifted its stay. Trojan began work on LCR on September 20, 1995. However, later in the day on September 20, 1995, the US Court of Appeals for the 9th Circuit issued a stay effectively stopping LCR activities again. Oral arguments are currently scheduled for September 28. Before the court actions, Trojan had removed two of its steam generators from containment and they are currently lying on their side in a covered warehouse onsite. A third steam generator had been lifted and placed on a rail system, still inside containment. Neither the fourth steam generator nor the pressurizer have been lifted from their cradles inside containment. Regional Action: The Region will continue to coordinate with NRR in monitoring the licensee's actions to ensure that the steam generators are in a configuration that provides a reasonable level of safety for workers and the public.
