Subject: LOST NICKEL-63 SOURCE                                           

Discussion:

On December 20, 1995, AT&T Richmond notified Region II that a            
generally-licensed 15 millicurie Nickel-63 source had been lost from a   
laboratory at the facility.  The source was in a thickness measuring     
device used in a research laboratory.  The source was present in December
1994, when it was inventoried and leak tested and subsequently determined
to be missing during the December 1995 inventory.  The licensee's        
investigation indicated that the source may have been lost between       
December 1994 and June 1995.  A physical search of the facility by the   
licensee did not locate the source.                                      

Regional Action:

Region II will conduct an inspection to evaluate the facility's control  
of material.                                                             
                                                                         
The Commonwealth of Virginia has been informed.
