MR Number: 1-95-0156 Calvert Cliffs 1 2 Date: 12/18/95 Lusby,Maryland SRI PC Dockets: 50-317,50-318 PWR/CE,PWR/CE Subject: MANAGEMENT CHANGES Discussion: On December 15, 1995, Baltimore Gas and Electric Company announced the following management personnel promotions and changes: Mr. Edward Crooke, the current President and Chief Operating Officer (COO), has been named Chairman of the Board of Constellation Holdings and will work full time on the future merger with Potomac Electric Power Company; Mr. George Creel, the current Senior Vice President - Generation, will be promoted to Executive Vice President and Acting COO and will replace Mr. Crooke; Mr. Robert Denton, the current Vice President - Nuclear Energy, will be promoted to Senior Vice President - Generation; Mr. Charles Cruse, the current Calvert Cliffs Nuclear Power Plant (CCNPP) Plant General Manager, will be promoted to Vice President - Nuclear Energy; Mr. Peter Katz, the current Nuclear Engineering Manager, will become the new CCNPP Plant General Manager; and Mr. Peter Chabot, the current Superintendent - Technical Support, will be promoted to Nuclear Engineering Manager. Contact: Lawrence Doerflein (610)337-5378 _ REGION III MORNING REPORT PAGE 2 DECEMBER 18, 1995 Licensee/Facility: Notification: MR Number: 3-95-0189 St. Joseph Hospital Date: 12/18/95 Elgin,Illinois VIA TELEPHONE License No: IL-01268-01 Subject: REMOVAL OF THERAPY IMPLANT BY PATIENT Discussion: On December 15, 1995, the Illinois Department of Nuclear Safety informed the NRC Region III office that a patient removed an iridium-192 implant from his nose. The implant consisted of a catheter containing five iridium-192 seeds with a total activity of 22.9 millicuries (618.3 millibecquerels). Apparently, the patient became disoriented during the early hours of December 15, 1995, and removed the catheter from his nose and dropped it on the floor. A nurse noticed the catheter upon entering room and immediately placed it into a shielded container. The patient received 855 rads (8.55 Gray), less than the intended dosage of 1500 rads (15 Gray). This was the second treatment of the patient's therapy plan. The patient and patient's physician have been contacted. The referring physician and oncologist are evaluating the treatment to determine if further therapy is necessary. The licensee will submit a 15-day written report to the Illinois Department of Nuclear Safety. The state agency is following the licensee's activities in this case. The Office of State Programs and NMSS have been notified. Regional Action: The Region III State Agreements Officer will monitor the Illinois Department of Nuclear Safety's followup action. MR Number: 3-95-0190 Zion 1 Date: 12/17/95 Zion,Illinois SRI VIA PC Dockets: 50-295 PWR/W-4-LP Subject: UNIT 1 STARTUP FOLLOWING COMPLETION OF REFUELING OUTAGE Discussion: On December 17, 1995, Unit 1 synchronized to the grid following a 99-day refueling outage. The outage was originally scheduled for 64 days. The major contributors extending the outage were: (1) extensive repairs to all four steam generators; and, (2) emergent work on Unit 2 which required resources to be diverted from outage activities. Carson, Co Date: 12/15/95 Rock Island,Illinois VIA TELECON Dockets: 03014796 License No: 12-00722-07 Subject: BURIAL OF 378 PROMETHIUM-147 SIGHTING DEVICES Discussion: The licensee reported the loss of 378 light anti-tank weapon sights which were last located at Fort Carson, Colorado. The licensee believes the weapon sights were inadvertently disposed of as waste in a landfill within the Fort Carson fenced area sometime between August 1994 and November 1995. The loss was discovered on December 15, 1995. Each sight was manufactured in 1979 or earlier and originally contained 3 millicuries of promethium-147. The promethium is encapsulated in a ceramic sphere and the sphere is laminated between two pieces of plastic. Based on a 2.6 year half- life and assuming that all of the sights were manufactured in 1979, the licensee's conservative estimate is that 0.016 curies of promethium-147 were buried at Fort Carson.
