Date: 11/17/95 Kirtland,Ohio LICENSEE NOTIFIED NRC BY TELEPHONE Dockets: 03020479 License No: 34-21301-01 Subject: MOISTURE DENSITY GAUGE DAMAGED BY HEAVY EQUIPMENT Discussion: On November 16, 1995 at approximately 4:15 p.m., a Troxler moisture density gauge, Model No. 3411B; Serial No. 11915, was damaged by soil compactor at a construction site in Garfield Heights, Ohio. The gauge contained a nominal 8.1 millicurie (0.3 GBq) cesium-137 sealed source and a 40 millicurie (1.48 GBq) americium-241 sealed source. After completing measurements with the gauge in a basement excavation project, the gauge technician walked approximately 40 feet away from the gauge to the site superintendent. During that time, a soil compactor which was operating in the area ran over the gauge. The housing unit was damaged and the source rod was bent; however, the sources remained intact and shielded within the device. A gauge technician cleared an area around the gauge and called the Radiation Safety Officer (RSO). The RSO contacted the Ohio Emergency Management Agency who also responded to the incident. Based on preliminary surveys done by a representative from the Ohio Emergency Management Agency, it appears that the sources remained intact inside the device in the shielded position. A leak test of the sources was taken but results were not available at this time. After the radiation survey and leak test wipe, the gauge was placed in its Type A shipping container and transported back and secured at the licensee's facility in Kirtland, Ohio. The licensee will voluntarily be issuing a report to the NRC. The licensee notified the State of Ohio of this event at approximately 3:30 p.m. (CDT) on November 16, 1995, and notified Region III at 7:15 a.m. (CDT) on November 17, 1995. This information is current as of 9:30 a.m. (CDT) on November 17, 1995. Regional Action: NRC Region III (Chicago) will conduct a special inspection to review the circumstances surrounding this event. HOLT (708)829-9836 _ HEADQUARTERS MORNING REPORT PAGE 2 NOVEMBER 17, 1995 Licensee/Facility: Notification: MR Number: H-95-0139 All Licensees Date: 11/17/95 Subject: Federal Response During Furlough Discussion: AEOD/ERB contacted FEMA, DOE and EPA to determine the Federal response capability in the event of a radiological emergency. The results of these contacts indicate that the Federal response can be activated promptly in the event of a radiological emergency. FEMA is furloughed. The NECC (the 24-hour notification point at Mt. Weather) is fully operational as are the five MERS centers (the 24-hour notification points for the Regions). All activation procedures will be used in the event of an emergency and staff can respond. DOE is not furloughed and all FRMAC and RAP capabilities are available as usual. EPA is furloughed. The National Response Center (the EPA 24-hour notification point) is fully operational.
