MR Number: 1-95-0129 Indian Point 2 Date: 10/10/95 Buchanan,New York SRI Dockets: 50-247 PWR/W-4-LP Subject: TECH SPEC SHUTDOWN Reportable Event Number: 29432 Discussion: On October 9, 1995, the Indian Point 2 reactor was taken to hot shutdown in accordance with the unit's technical specifications (TS) due to the inoperability of safety-related equipment. Specifically, on October 5, 1995, a 14 day limiting condition for operation (LCO) per TS 3.3.E.2.a was entered as a result of maintenance on the 21 component cooling water (CCW) pump, one of three CCW pumps. On October 9, 1995, operators identified a level increase in the lube oil level for the 23 emergency diesel generator (EDG) believed to be the result of service water leakage through the lube oil cooler heat exchanger. Testing of the lube oil confirmed the presence of water in the oil and the 23 EDG was declared inoperable. TS LCO 3.7.B.1 allows one EDG to be out of service for 7 days provided offsite electrical sources are available and the remaining EDGs and the engineered safety features associated with these EDG buses are operable. Con Edison was unable to meet this requirement as the 21 CCW pump was still inoperable and is powered off of the 21 EDG emergency bus. Therefore, TS 3.0.1 was entered at 0245 on October 9, 1995. TS 3.0.1 states in part that the unit shall be placed in at least hot shutdown within 7 hours and within cold shutdown within the following 30 hours unless corrective measures are completed that restore compliance to the LCO. Efforts to restore either the 23 EDG or 21 CCW pump during the seven hour shutdown period were unsuccessful and the reactor trip breakers were opened with the reactor already subcritical at 0944. Following the shutdown, Con Edison pursued efforts to return either the 21 CCW pump or the 23 EDG to operability prior to having to take the unit to cold shutdown. At 2350, the 23 EDG was returned to service, after the identification and plugging of a leaking tube, and TS 3.0.1 was exited. At 0510 on October 10, 1995, the 21 CCW pump was declared operable. The licensee is making preparations for restart of the facility. Regional Action: The resident inspectors responded to the site on October 9 and monitored the licensee's activities in response to the problems. Of particular followup interest is the fact that this is the second time in less than two weeks that the 23 EDG experienced leakage through the lube oil heat exchanger that resulted in the EDG being declared inoperable. MR Number: 4-95-0125 South Texas 2 Date: 10/10/95 Wadsworth,Texas Resident Inspector Dockets: 50-499 PWR/W-4-LP Subject: UNIT SHUTDOWN FOR FOURTH REFUELING OUTAGE Discussion: On October 7, 1995, at 12 a.m. (CDT), Unit 2 shut down to begin its fourth refueling outage. The outage is scheduled to last 34 days. Major outage activities include 100 percent eddy current testing and sludge lancing of steam generators. Major secondary plant work includes significant inspection and replacement of the rotors in the main turbine due to previously identified cracking. MR Number: 4-95-0126 Callaway 1 Date: 10/10/95 Fulton,Missouri Resident Inspector via LAN Dockets: 50-483 PWR/W-4-LP Subject: INADVERTENT LOSS OF 700 GALLONS OF REACTOR COOLANT Discussion: At 4:10 a.m. on October 9, 1995, during maintenance on the Reactor Coolant Pump (RCP) B, with the reactor in Mode 5, the licensee inadvertently drained approximately 700 gallons of water from the reactor coolant system (RCS) to a reactor building sump. At 4:30 a.m., the leak was terminated. The apparent cause of the event was an inadequate maintenance procedure. During work on the RCP, the procedure required the installation of a temporary modification on the RCP to allow draining of the pump cavity. However, it did not require removal of the temporary modification prior to restoration from maintenance. As a result of this event, the licensee established a team to review the RCP maintenance procedures for additional problems. The licensee's engineering organization will also determine if a permanent modification will be installed. MR Number: 4-95-0127 Callaway 1 Date: 10/10/95 Fulton,Missouri Resident Inspector via LAN Dockets: 50-483 PWR/W-4-LP Subject: POTENTIAL GENERIC MATERIAL CONDITION DEFICIENCIES Discussion: The licensee identified a potential manufacturing defect in the Reactor Coolant Pump 1 seal housing bolts. A small burr was left in the bottom of the center hole, which affected the bolt stretch measurements. The bolts were not stretched the required amount, which allowed the seal housing to pump casing gasket to leak. The licensee replaced the bolts with studs that allowed easier measurement of the required stretch. On a separate issue, during the recent forced outage, the licensee noted a problem with residual heat removal (RHR) flow to reactor coolant system Cold Leg D. This line is also the low pressure safety injection line. Upon investigation, the licensee determined that a check valve disk had fallen out of its retaining ring. There appeared to be excessive wear on both the disk and the ring. These check valves are on a 3-year inspection program and had been replaced in the refueling outage earlier this year. The licensee determined that the size of the retaining ring was incorrect. Due to flow turbulence, the disk rotated inside the ring. After sufficient material had worn away, the disk dropped into the flow path. The licensee replaced the disk and ring. The licensee performed a safety evaluation and determined that the turbulence in the Cold Leg D injection line was unique. Also, the replacement disk and ring were of the correct dimensions. Therefore, the licensee is proceeding with reactor startup. The licensee is investigating these issues to determine if there is a Part 21 reporting requirement.
