The licensee reported that on November 7, 1995, the Troxler Model 3430 moisture-density gauge had been recovered intact by the East Cleveland Fire Department. The gauge was in the transportation overpack and the overpack and gauge were found unlocked, however the sealed source was in the shielded position. On November 8, 1995, the licensee reported that a leak test was performed following recovery of the gauge and the results are pending. The licensee will submit a written report within 30 days. MR Number: 3-95-0167 Monticello 1 Date: 11/09/95 Monticello,Minnesota RESIDENT INSPECTOR Dockets: 50-263 BWR/GE-3 Subject: EMERGENCY DIESEL INOPERABLE Discussion: On 11/7/95 the licensee identified speed control problems with the No. 12 Emergency Diesel Generator (EDG) governer and entered into an LCO to replace the governer. On 11/8/95, after replacement of the governer, the licensee operated the EDG but experienced problems during the shutdown sequence. Instead of the EDG stopping at the end of an eleven minute cooldown period, the EDG ran up to full speed. Operators attempted to lower the speed from the control room but were unsuccessful. Operators stopped the EDG locally by tripping the fuel racks. The licensee determined during operation of the EDG that the governer motor received an increase signal (to reset the governer to 100 percent for subsequent start) concurrent with the operators sending a decrease signal (to lower EDG speed). The governer motor failed as a result of having both contactors closed simultaneously in excess of 3 minutes. The licensee attributed the failure as a reversal of polarity on the shutdown solenoid. Engineering identified non-standard station wiring to the governer. The licensee received the governer as a like-for-like replacement. However, this newer model had a half wave rectifier circuit installed. The removed governer did not have this feature. This made the shutdown solenoid on the new governer incompatible with existing station wiring. The licensee replaced the motor and shutdown solenoid from the previously removed governer. Installation should completed on 11/8/95 and testing on 11/9/95. As a long term solution, the licensee planned to implement a modification to permanently change the station wiring to both EDG governers since all newer governer models have half wave rectifiers installed.
