PHARR (404)331-5903 _ REGION II MORNING REPORT PAGE 3 SEPTEMBER 8, 1995 Licensee/Facility: Notification: MR Number: 2-95-0082 Veteran's Affairs Medical Center Date: 09/08/95 Lexington,Kentucky Dockets: 03010227 License No: 16-08896-04 Subject: BRACHYTHERAPHY MISADMINISTRATION Discussion: Referencing to MR Number 2-95-0079, on September 8, 1995, the licensee's Radiation Safety Officer (RSO) notified Region II of a brachytherapy misadministration. The licensee stated that eleven iridium-192 seeds totalling 58.9 millicuries were reimplanted at 3:35 p.m., September 7, 1995, to resume the bronchial treatment. Sometime between 3:35 a.m. and 4:30 a.m., September 8, 1995, the patient, who was in restraints, managed to remove the restraints and removed the catheter containing the seeds. At 4:30 a.m., nursing personnel observed that the catheter had been removed and was on the nightstand near the patient's bed. The nurse immediately removed the patient from the room, secured the room, which is a corner room on the hospital's fifth floor, and contacted the RSO. The maximum exposure recorded on nursing dosimeters was 10 millirem (mrem). The patient admits having removed the catheter. The licensee's investigation is continuing. Reimplantation of the sources is not planned at this time. Regional Action: Region II is evaluating the potential radiation doses to individuals. MR Number: 4-95-0110 Trojan 1 Date: 09/08/95 Ranier,Oregon BY LICENSEE BY PHONE Dockets: 50-344 PWR/W-4-LP Subject: STATE COURT ORDERED STAY TO STOP LARGE COMPONENT REMOVAL Discussion: On September 7, 1995, the licensee informed Region IV and NRR that the Supreme Court of the State of Oregon issued a stay to Portland General Electric (PGE) which halted all activities related to Trojan's Large Component Removal (LCR) plan. Trojan's LCR plan involved removing all four of its steam generators and its pressurizer, and barging those components to U.S. Ecology's disposal facility in Hanford, Washington. These activities were conducted as part of Trojan's ultimate decommissioning. Don't Waste Oregon Council (DWOC) filed its law suits in court on September 5, after receiving information that NRC will not appeal the decision rendered by the First Circuit Court of Appeals regarding the decommissioning at Yankee Rowe. Before the court action, Trojan had removed two of its steam generators from containment and they are currently lying on their side in a covered warehouse onsite. A third steam generator had been lifted and placed on a rail system, still inside containment. Neither the fourth steam generator nor the pressurizer have been lifted from their cradles inside containment. Region IV has had weekly inspector coverage at Trojan since preparations were underway for lifting the first steam generator, during the week of July 31. The first steam generator was lifted out of its cradle in containment during the week of August 21. The Region IV Regional Administrator conducted a scheduled site tour at Trojan during the afternoon of September 7. REGIONAL ACTION: The Region will continue to coordinate with NRR in monitoring the licensee's actions to ensure that the steam generators are in a configuration that provides a reasonable level of safety for workers and the public.
