Headquarters Daily Report NOVEMBER 07, 1995 *************************************************************************** REPORT NEGATIVE NO INPUT ATTACHED INPUT RECEIVED RECEIVED HEADQUARTERS û REGION I û REGION II û REGION III û REGION IV û PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION I NOV. 07, 1995 Licensee/Facility: Notification: New York Power Authority MR Number: 1-95-0140 Indian Point 3 Date: 11/07/95 Buchanan,New York SRI PC Dockets: 50-286 PWR/W-4-LP Subject: FAILURE OF THE APPENDIX-R EMERGENCY DIESEL GENERATOR Discussion: On November 3, 1995, during the monthly performance test of the Appendix R emergency diesel generator (EDG), the EDG failed to function properly. Approximately 50 minutes into the 60 minute run, the output decreased from 2500 KW to 500 KW. The operator unsuccessfully attempted to control the output with the governor control switch. Load then increased to approximately 1000 KW and then dropped to 0 KW. The operator opened the output breaker and secured the machine. The operator reported hearing a loud pop and mechanical sounds during the event. Troubleshooting revealed the turbocharger had seized. Further inspection identified a broken rotor blade, indications of rubbing of the turbine and compressor blades on the case, and indications of bearing material in the oil. The exact sequence of the damage is still under investigation by the turbocharger vendor. NYPA is currently evaluating options to repair or replace the turbocharger. Additionally, engine inspections, including internal inspection of the cylinders, is planned. The Appendix-R EDG is an ALCO model 18-251, rated at 2500 KW. The EDG is just over 10 years old with only 108 hours of run time on the engine.
