Headquarters Daily Report OCTOBER 03, 1995 *************************************************************************** REPORT NEGATIVE NO INPUT ATTACHED INPUT RECEIVED RECEIVED HEADQUARTERS û REGION I û REGION II û REGION III û REGION IV û PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION III OCTOBER 3, 1995 Licensee/Facility: Notification: Indiana MR Number: 3-95-0155 Cook 1 Date: 10/03/95 Bridgman,Michigan Dockets: 50-315 PWR/W-4-LP Subject: DAMAGE FUEL ASSEMBLE Discussion: On September 29, 1995, at 2:50 a.m. (EST), a new fuel assembly became pinched between the upender and the transfer canal wall in the spent fuel pool area. The licensee had performed a visual inspection of the bottom nozzle of fuel assembly GG10 using an underwater camera. However, to perform this inspection, the assembly was lowered to obtain better focus with the camera on the bottom nozzle. After this inspection, the assembly was not raised to the normal height of an assembly prior to the upender being raised to a vertical position for placing the assembly in the upender. As a result, when being raised to the vertical position, the upender contacted the lower portion of the assembly and pinched the assembly up against the transfer canal wall. After lowering the upender on September 30, an inspection of the assembly identified several fuel pins that were displaced approximately 4 3/4 inches. The assembley does have a used control rod which the licensee wants to remove prior to sending the assembly offsite. Due to the damage, the assembley can not be placed in the new fuel elevator. The licensee presently plans to store the damaged assembly in an old sipping stand and then try to remove the control rod. There does not appear to be any rupture of fuel pins. Regional Action: The resident inspectors will monitor licensee's actions to place the assembly in a safe configuration and will review the licensee's corrective action. MR Number: 4-95-0123 Diablo Canyon 1 Date: 10/02/95 Avila Beach,California Call from Resident Inspector Dockets: 50-275 PWR/W-4-LP Subject: REACTOR COOLANT PUMP ELECTRICAL FAULT Discussion: On September 30, 1995, with Unit 1 in Mode 4 and shutting down for the start of Refueling Outage 1R7, an electrical fault and fire occurred in the containment penetration electrical connection box for Reactor Coolant Pump (RCP) 1-3. The fire was identified by plant staff and extinguished by securing RCP 1-3. The connection box is on the external portion of the RCP containment penetration (on the exterior portion of the containment building). Initially, RCPs 1-1 and 1-2 were operating for decay heat removal and pressure control. At 1:12 p.m. (PDT), high vibration on RCP 1-2 prompted operators to start RCP 1-3 and secure RCP 1-2. Immediately following the start of RCP 1-3, a phase-to-ground fault was identified by the control room operators. At approximately 1:14 p.m. (PDT), personnel in the vicinity of the Unit 1 north pipe rack area reported smoke. Operators quickly identified the source of the fire and secured RCP 1-3. The fire lasted for approximately 4 minutes and, therefore, was not reportable under the licensee's emergency plan. The licensee's inspection of the connection box found that the fault had destroyed a portion of one of the connectors inside the box (one phase) and burned a 5-inch diameter hole in the connection box cover. Initial inspections of the containment penetration have not revealed any degradation of its integrity. The penetration maintained a positive pressure of 55 psi throughout the event. The licensee is evaluating the potential damage to the penetration, cabling, and replacement of the connector.
