The licensee reported that on October 20, 1995, the Troxler Model 3411 moisture-density gauge had been recovered intact by construction workers in a waste collection area. The gauge was not in the transportation overpack, however, the handle was locked with the sealed source in the shielded position. On October 31, 1995, the licensee reported that a leak test performed following recovery of the gauge revealed no contamination. The licensee will submit a written report within 30 days. KOZAK (708)829-9866 _ REGION IV MORNING REPORT PAGE 4 NOVEMBER 1, 1995 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-95-0134 Cooper 1 Date: 11/01/95 Brownville,Nebraska Phone Call from SRI and Licensee Dockets: 50-298 BWR/GE-4 Subject: INADVERTENT TRANSFER OF REFUELING WATER TO RADWASTE AND TEMPORARY LOSS OF SPENT FUEL STORAGE POOL COOLING Discussion: On October 31, 1995, Cooper Nuclear Station (CNS) inadvertently transferred approximately 10,000 gallons of refueling water to the low level radwaste system, resulting in a trip of the Fuel Storage Pool (FSP) cooling water pumps on low suction pressure. At the time, the reactor cavity was flooded-up, fuel gates were open, and all fuel was offloaded into the FSP. The event occurred at approximately 5:10 a.m., when a cable from the shroud inspection tracker (remote camera), being positioned by a contractor, contacted and inadvertently opened a 1.5-inch ball valve in the main steam line plug vent line allowing refueling water to flow into the "C" main steam line (MSL). From there, the water flowed through the MSL drain lines to equipment drain sump and, finally, to the radwaste system. At approximately 5:30 a.m., operators received a low level alarm in the skimmer surge tank, and proceeded to provide make-up. The makeup transfer was not sufficient to overcome the level decrease in the surge tank, and FSP Pumps A and B tripped on low suction pressure at 6:26 a.m. and 6:44 a.m. Operators were able to restart FSP Pumps A and B at 6:55 a.m. and 6:47 a.m., respectively. Radwaste personnel notified the control room at approximately 6:00 a.m. of increased radwaste collection volume, which facilitated the identification of the flowpath. The transfer was terminated at 7:15 a.m., when operators shut the MSL drain line. The MSL plug vent line was secured at 7:30 a.m. FSP level dropped approximately 1.5 inches. Cooling to the FSP was lost for approximately 3 minutes, however, FSP temperature remained constant at 117 degrees F. The transfer was contained in the MSL, the equipment drain sumps, and the radwaste system. The licensee stopped work on the refueling floor for 8 hours to review this event. Regional Action: Resident inspectors will evaluate the licensee's planning and control of this evolution, its safety significance, and the licensee's corrective actions. Contact: T.
