MR Number: 1-95-0106 Susquehanna 2 Date: 08/23/95 Allentown,Pennsylvania RI PC Dockets: 50-388 BWR/GE-4 Subject: SUSQUEHANNA UNIT 2 NEW FUEL BUNDLE DROPPED Reportable Event Number: N/A Discussion: On August 22, 1995, while performing a fuel shuffle from the Unit 2 fuel vault to the fuel preparation machine, a new fuel bundle fell into the fuel preparation machine in the spent fuel pool when the grapple separated from the hoist cable. The bundle was being lowered into the machine at the time of the event and the bundle fell approximately 15-20 feet through water until it impacted the lower carriage support plate. The bundle is currently in the fuel preparation machine at a slight angle (approximately 10 degrees) with the nose of the bundle resting to one side of its desired location. The licensee has secured the bundle to prevent further movement. There were no radiological consequences or personnel injuries. The bundle is positioned relatively far from the spent fuel bundles in the pool. No physical damage to the fuel bundle is apparent. The licensee has consulted the fuel vendor and is developing a recovery plan. A licensee Event Review Team has also been initiated. The cause of the grapple separation from the hoist cable is currently unknown. All fuel movements in the pool have been stopped pending determination of the root cause of this event and any necessary corrective actions. MR Number: 4-95-0098 Waterford 3 Date: 08/23/95 Killona,Louisiana PHONE CALL FROM RESIDENT INSPECTOR Dockets: 50-382 PWR/CE Subject: IMPROPER FIRE WATCH PERFORMANCE AT WATERFORD 3 Reportable Event Number: N/A Discussion: On July 28, 1995, the licensee's corporate and site security managers received an anonymous phone call regarding fire watch improprieties by several contract security guards at Waterford-3. The licensee initiated an internal security investigation and determined on August 2 that one of the named individuals admitted to not following licensee procedures. The failure involved taking credit for a fire watch tour that was performed by someone other than the individual signing the fire watch log. However, the security department did not initiate a Condition Report (the licensee's corrective action document) until August 17th, when the licensee completed the investigation of all the named individuals. As a result of the investigation, the contract security guard has been terminated. Unrelated to the first event, on August 18th a licensee Quality Assurance auditor determined, by direct observation, that another contract security guard had apparently falsified a fire watch log, in that the fire watch of a ventilation room in the reactor auxiliary building was not performed even though the log had been signed indicating satisfactory completion. These quality assurance audits were implemented as a corrective action for a NRC-identified violation involving inadequate fire watches in March 1993. This latter incident is still under investigation and evaluation. Because the security department had not initiated a corrective action document until they had completed their investigation on August 17th, licensee management and the resident inspectors were not informed of the problem until August 18th. On August 22nd, the resident inspectors, and regional and NRR management, participated in a teleconference with the licensee to more fully understand the issues, the timeliness of condition report initiation and NRC notification, and the licensee's corrective actions. The short-term corrective actions include: pre-watch briefings of the contractor security personnel who perform fire watches, increased in-plant observation by security supervisors, continued quality assurance audits of fire watches, a significant root cause analysis of security performance, and an immediate evaluation of the timeliness of condition report initiation. Longer term considerations may involve the use of "night watchman clocks" throughout the plant. Regional Action: This morning report is being submitted for information only. Contact: C. A.
