MR Number: 3-95-0144 Palisades 1 Date: 08/17/95 Covert,Michigan RESIDENT Dockets: 50-255 PWR/CE Subject: CONTROL ROD WITHDRAWL WHEN GIVEN AN INSERTION DEMAND Reportable Event Number: N/A Discussion: On August 17, 1995, during low power physics testing following initial criticality for cycle 12, an unexpected control rod motion occurred. Group 4 rods had been pulled as part of the first phase of low power physics testing. When reinserting the group 4 rods to their original position, group 4 rods no. 40 and no. 41 both moved out upon an insertion demand. Rod control was in manual sequential. Plant operators placed rod control to manual individual and rod no. 41 inserted; however, rod no. 40 continued to rise when given an insertion demand. The licensee declared rod no. 40 inoperable and commenced boration to subcritical conditions (critical boron concentration with all rods out plus 100 ppm). Plant operators later tripped the reactor when initial troubleshooting of the as-found condition identified a short circuit between the lower and raise control rod circuitry. The plant is currently in hot shutdown with the investigation continuing. Regional Action: Region III dispatched an electrical engineer to assist the resident in review of this event. Contact: D.G. MR Number: 3-95-0145 Quad Cities 1 2 Date: 08/18/95 Cordova,Illinois RI VIA LAN Dockets: 50-254,50-265 BWR/GE-3,BWR/GE-3 Subject: STATUP OF UNIT 2 FROM UPLANNED OUTAGE DUE TO RELIEF VALVE "WEEPAGE" AND ELECTRO-HYDRAULIC CONTROL (EHC) SYSTEM LEAKS. Reportable Event Number: N/A Discussion: On August 18, 1995, at 4:36 a.m., the Unit 2 main turbine was synchronized to the grid. Unit 2 was shut down on August 13 to address elevated downstream tail pipe temperatures on four power operated relief valves (PORV) and repair a leak in the EHC system. The licensee planed to increase power to about 50 percent through the weekend and then continue refueling outage startup testing. The licensee identified indications of steam leakage from the main seat on the "B" and "E" PORVs and from the "B" pilot seat. No indication of seat damage was identified. The licensee removed a portion of an abandoned angle iron near the tail pipe of the "E" valve. While no definitive root cause was determined, the licensee speculated that thermal growth of the tail pipe may have contributed to seat distortion. The EHC leak was from the accumulator shutoff assembly on the #6 combined intercept valve. The shutoff assemble was replaced. The other assemblies installed during the refueling outage were inspected and no other leakage was identified. The cause of the leak was incorrect bolt size and gasket material used in the leaking shutoff assembly. The accumulator assemblies installed in Unit 1 during the last outage have performed without problems. Additional Followup: The "B" train of control room ventilation failed the monthly surveillance test on August 12 and was declared inoperable. The cause was a leak in the freon system. The leak was repaired and the system retested and returned to service on August 16. Regional Action: Resident inspectors continue to monitor the licensee's restart testing program. Contact: P.L.
