MR Number: 2-95-0070 Oconee 3 Date: 08/14/95 Seneca,South Carolina Dockets: 50-287 PWR/B&W-L-LP Subject: OCONEE UNIT 3 REACTOR TRIP Reportable Event Number: 29194 Discussion: This is an update to the August 14, 1995, reactor trip event at Oconee Unit 3. The Group 5 control rods dropped into the core at 4:56 a.m. causing a variable low pressure reactor trip from 100 percent power. The trip was without complications, with all safety systems functioning as required. The licensee's investigation into the cause of the dropped Group 5 control rods led to the replacement of the Group 5 control rod drive programmer. As a specific problem with the programmer could not be identified, the licensee also installed recorders on the system's power supplies for future monitoring. Unit 3 was restarted on August 15, 1995, at 2:16 a.m., and is currently undergoing power escalation. Regional Action: The resident inspector responded onsite to the trip and will continue to followup, including the repetitive nature of this Group 5 control rod drop. Date: 08/14/95 Chicago,Illinois TELEPHONE CALL FROM THE LICENSEE Dockets: 03004192 License No: 12-10243-01 Subject: LOSS OF TWO 15 MILLICURIE NI-63 ELECTRON CAPTURED DEVICES (ECDS) FROM A TRACOR GAS CHROMATOGRAPH. Reportable Event Number: N/A Discussion: On August 14, 1995, U.S. EPA Central Regional Laboratory reported the loss of two 15 millicurie (mCi) (555 megabecquerel) Ni-63 Electron Capture Devices (ECDs) from a Tracor Gas Chromatograph (GC) unit. The identification that the licensee had lost these sources was prompted during a licensing review and a subsequent interview of the licensee's Radiation Safety Officer (RSO). The RSO believed that his predecessor had properly disposed or transferred the GC unit containing the two ECDs, but his recent investigation of this incident revealed that the GC unit containing the Ni-63 sources was still active on his license as indicated by the manufacturer's (Tracor) source tracking list. The RSO investigation revealed that on June 25, 1992, the GC unit containing two 15 mCi (555 megabecquerel) Ni-63 sources was listed on a government excess list for a potential sale or transfer, and on August 12, 1992, the GC unit was picked up by the Government Services Administration (GSA). The RSO indicated that there were no records that showed the unit was sold or transferred for proper disposal, and he concluded that the GC unit containing these two Ni-63 ECD sources was disposed to a scrap metal dealer. The RSO stated that since 1993, a new policy was enacted to ensure that ECDs (Ni-63 sources) were removed prior the disposal of GC units.
