Athos Road (State route 726) outside the NNFD plant this morning. One sign read "August 9, 1945" and the other sign read "A fire we can't forget." The protestors were from the "Naval Nuclear Action Group". This group used to have pickets along Route 726 the first Wednesday of each month but have not picketed in over a year. A local television station (WSET-TV, channel 13) had a film crew taking pictures of the protestors. NNFD declined to comment or provide anyone to appear "on camera." The Sr. Resident Inspector is on site and will follow any further developments. In the past, the protestors have remained on the road and have not attempted to approach the facility or workers. MR Number: 3-95-0132 Davis Besse 1 Date: 08/09/95 Oak Harbor,Ohio VIA E-MAIL Dockets: 50-346 PWR/B&W-R-LP Subject: PLANT TRANSIENT WHILE REPLACING EHC INDICATING LAMP Reportable Event Number: N/A Discussion: On August 7, 1995, with the plant at 100 percent power, the unit experienced a 15 percent power transient that occurred during a 45-second period. During performance of a lamp test of the General Electric Turbine Generator EHC control panel in the control room, one of the indicating lights on a portion of the panel that had been labelled as "disabled" failed the lamp test. This portion of the EHC circuit was a Main Turbine Inlet Throttle Pressure Limiter. It was abandoned in place by an engineering change incorporated in 1987 with the "OFF" light energized and the reference level potentiometer set to 0 percent of scale. The light that failed the lamp test was the "ON" light. A decision was made by shift personnel to replace the faulty "ON" light. While replacing the light, the Turbine Control Valves started to rapidly shut, initiating a plant transient. The operator took immediate action by pressing the "OFF" button on the Throttle Pressure Limiter and the Turbine Control Valves thereafter reopened to their normal position. As a result of the Turbine Control Valve rapidly shutting, a Primary to Secondary heat imbalance caused reactor pressure to increase from a nominal value of 2155 to 2245 psig. Steam Generator pressure increased from a nominal value of 870 to 950 psig causing the Turbine Bypass Valves to open. The duration of the transient, from initiation until plant parameters returned to nominal values, was about 45 seconds. All primary and secondary systems responded to the transient as designed. The licensee has placed "do not operate" tags on the Throttle Pressure Limiter and is evaluating the electrical configuration. MR Number: 3-95-0133 Zion 1 Date: 08/09/95 Zion,Illinois SRI VIA PC Dockets: 50-295 PWR/W-4-LP Subject: TESTING OF NEW VOLTAGE REGULATOR FOR THE 1A EMERGENCY DIESEL GENERATOR (EDG) Reportable Event Number: N/A Discussion: On August 6, 1995, the 1A EDG experienced voltage fluctuations during a technical specification (TS) required seven day test. The test PT-11, " 1A Diesel Generator Loading Test," slowly loads the EDG to 4 megawatts. Troubleshooting revealed that the EDG voltage regulator had failed. The voltage regulator was replaced on August 7. The licensee initially intended to reperform PT-11 as the post maintenance test. This test has been routinely performed to verify EDG operability after any EDG repairs. As a result of the repairs to the 1A EDG, corporate engineering performed a review of the PMT to verify that the PMT adequately tested the functionality of the new voltage regulator. The review determined that the PMT (PT-11) did not adequately test the voltage regulator. The licensee developed a new PMT, TSSP-065-95, "1A EDG Testing after Voltage Regulator Replacement," to test the voltage regulator by ensuring minimum and maximum voltage levels were maintained after starting and stopping large safety related loads. The testing required deenergization of the associated safety related bus, and voluntary entry into a technical specification which permitted power operation for a maximum of four hours.
