MR Number: 3-95-0130 Quad Cities 2 Date: 08/07/95 Cordova,Illinois AUGUST 6, 1995, E-MAIL Dockets: 50-265 BWR/GE-3 Subject: START-UP OF QUAD CITIES UNIT 2 AFTER REFUEL OUTAGE. Reportable Event Number: N/A Discussion: On July 20, 1995, on day 138 of the refuel outage, operators made Unit 2 critical. With Unit 2 in start-up mode at about 2 percent reactor power, the 2B reactor recirculation pump spuriously increased in speed from about 32 percent to 60 percent. Reactor power increased to about 3 percent prior to operators manually reducing recirculation speed. The licensee's root cause investigation points toward a noise induced transient caused by feedback from inputs to a ribbon cable on a data acquisition system. The close proximity of the inputs on the ribbon cable allowed input with a "noisy" channel (reactor recirculation speed signal) to affect reactor recirculation demand signal and reactor level channels. On July 25, with the reactor in startup mode and power at about 3-4 percent, and pressure about 470 psig with pressure set in control, the turbine bypass valves (TBVs) started to oscillate. The oscillations lasted about 30 seconds and involved 3 TBVs. Power changed by about 1 percent and level changed by a few inches. Operators took control using the jack function on the turbine control panel. Operators shut down Unit 2 on July 26 at 4:50 a.m. to repair turbine pressure regulator circuits. Unit 2 was made critical again on July 27. However, at 920 psig TBV oscillations recurred. Operators shut down Unit 2 on July 29 to make repairs. The licensee determined that TBV oscillations were due to improper incorporation of GE SIL recommendations. Subsequently the licensee implemented a modification which installed new low-pass filters and notch filters in the turbine pressure regulator circuits as recommended by GE SIL 589. On August 2, operators made Unit 2 critical. On August 4, day 153 of the refuel outage, operators synchronized Unit 2 to the grid. The licensee was continuing with startup testing activities. Full power operations may occur about August 18. Regional Action: The resident inspectors were continuing to monitor start-up testing activities. Contact: P.L. MR Number: 3-95-0131 Fermi 2 Date: 08/07/95 Newport,Michigan TELEPHONE CALLS FROM LICENSEE. Dockets: 50-341 BWR/GE-4 Subject: DEMONSTRATIONS WITH ARRESTS. (EVENT DATES WERE AUGUST 5TH AND AUGUST 6TH, 1995. Reportable Event Number: N/A Discussion: A demonstration occurred at the Fermi 2 Owner Controlled Area (OCA) gate, located approximately one mile from the plant. The demonstration began at approximately 1:30 p.m. (EDST) and was completed about 3:45 p.m. An estimated 50 people participated. Three demonstrators used a ladder to scale the OCA fence and were detained on company property by site security officers. After declining a request to leave, they were turned over to the Monroe County Sheriff's Department who charged them with trespassing. A fourth person had been briefly detained earlier and released. A second demonstration took place on 8/6/95 from approximately 1:30 p.m. to 5:00 p.m. Approximately 60-65 people participated and there were no arrests. One person briefly chained himself to the OCA gate and released himself. Access to the facility was assured through alternate gates. There were no serious injuries, and plant operation was never disrupted. These demonstrations were part of a 10-day series of events in Michigan sponsored by a coalition of groups apposed to nuclear weapons and to nuclear plant operations.
