MR Number: 1-95-0095 Indian Point 2 Date: 07/25/95 Buchanan,New York Dockets: 50-247 PWR/W-4-LP Subject: UPDATE ON UNPLANNED RELEASE OF CONTAMINATED LIQUID (MR 1-95-003) Reportable Event Number: N/A Discussion: On the afternoon of July 18, 1995, an unplanned radiological liquid discharge occurred through the Indian Point 2 discharge canal into the Hudson River. The release occurred due to a valving error while restoring the fire protection water tank level control valve to service. Initial estimates of the discharge were on the order of 2400 gallons. During a review of blueprints of the drain systems, an additional flow path was identified. It is estimated that this additional path could have discharged as much as 1000 gallons per minute. Consolidated Edison performed a bounding calculation for 28,000 gallons discharged (based on the fire protection water storage tank overflowing for 35 minutes, and 7,000 gallons going to the liquid radwaste system), and has determined that the expected radiation dose to the most exposed individual is approximately equal to 2 minutes of natural background radiation (1.67 E-4 mRem whole body). Consolidated Edison continues to review the event and plans to inspect the storm drain system on-site with a robot and camera to identify why the water flowed into contaminated areas. MR Number: 2-95-0064 Robinson 2 Date: 07/24/95 Hartsville,South Carolina Dockets: 50-261 PWR/W-3-LP Subject: UE DUE TO RCS LEAKAGE GREATER THAN 10 GPM Reportable Event Number: 29101 Discussion: At approximately 3:00 p.m., on July 24, 1995, the licensee isolated normal letdown to perform scheduled maintenance on level control valve LCV-115. Excess letdown was placed in service and charging was reduced to match the excess letdown flow rate. Following this adjustment, the operators noted reduced seal injection flow to the B reactor coolant pump. In an effort to compensate, operators throttled down on valve HCV-121 thereby reducing charging flow and diverting more flow to the seals. At approximately the same time, the relief valve on the discharge of the C charging pump lifted and failed to totally reseat. (The A charging pump was running and the B and C pumps were idle.) An operator in the charging pump room also noted a small leak in the weld of the flanged connection of the A charging pump discharge relief valve. The licensee declared a UE at 1533 due to RCS leakage greater than 10 gpm. The A and C charging pumps were isolated which terminated the leak and the B charging pump was started. The licensee exited the UE at 1639. The plant entered a 24-hour LCO action statement due to having two inoperable charging pumps. At 0733 on July 25, 1995, the licensee exited the LCO. The licensee has replaced the pressure relief valve on the C charging pump. The A charging pump is still inoperable. Bill McCollum has been the Plant Manager at Catawba since 1991. The plant manager position will be filled by Gary Peterson, who joined DPC in December 1994 as General Manager, Nuclear Assessment and Issues. HEALTH Dockets: 03030376 License No: 21-25870-01 Subject: RECOVERY OF LOST MOISTURE/DENSITY GAUGE (TELECON FROM MICHIGAN DEPT. OF PUBLIC HEALTH) Reportable Event Number: N/A Discussion: The Michigan State Police (MSP) reported the recovery of a Troxler Model 3411B moisture/density gauge reported missing by the licensee on May 9, 1995, after use near their Saginaw, MI facility. The gauge had been found in a drainage ditch along a roadside by a private citizen about 3 weeks ago and placed in his garage until it was turned over to the MSP on July 24, 1995. The MSP reported the gauge was in good condition, still in its transportation case with the source rod handle padlocked in the closed position. The MSP used a Civil Defense Model 715 meter to survey the exterior of the transportation case and indicated no significant radiation readings. The licensee recovered the gauge from the MSP, performed a leak test and plans to return the gauge to the manufacturer to assure the gauge is safe for use. A preliminary direct survey of the leak test wipe using the MSP's CDV-715 indicated no contamination. Regional Action: NMSS was notified and followup is planned on a routine inspection.
