The physician, via a written directive, requested a dose of 100 microcuries of sodium iodine (I-131). The I-131 capsule received from the nuclear pharmacy contained approximately 60 microcuries. The capsule was not administered to the female patient, and another capsule with the proper dose was requested and received from the nuclear pharmacy. Both capsules (approximately 160 microcuries) were given to the patient in error. The misadministration was discovered during the review of the administration on July 19, 1995. The licensee has begun the investigation of the incident. Dosimetry calculations by the licensee estimate the dose to the thyroid to be approximately 600 cGy. The licensee does not expect adverse health effects to the patient. The licensee notified the referring physician, but efforts to notify the patient have been unsuccessful thus far. Regional Action: Region II will conduct a followup inspection the week of July 24, 1995, and has notified the Commonwealth of Virginia. Bill Campbell, currently Vice President-Nuclear Engineering, has been named Vice President-Brunswick Nuclear Plant, succeeding Anderson. Bill Habermeyer, currently Vice President-Nuclear Services and Environmental Support, has been named Vice President-Nuclear Engineering, succeeding Campbell.
