The loss of scram function was discovered during the quarterly scram checks. A float type device was prevented from activating due to the presence of what appeared to be wire insulation. There was no indication of loss of primary outlet flow and the scram function has been repaired and tested. According to the licensee, the insulation was not of a type used around the reactor. If the piece of insulation had blocked flow through one of the fuel assembly channels, the licensee stated that they would have seen indication of a higher than normal temperature in one of the fuel boxes. The licensee plans to take the issue to the safety review committee on June 13 together with planned actions for review and approval prior to restart. Regional Action: The region will evaluate the licensee's proposed actions following their formal establishment by the safety committee. Followup inspection will be during the next routine inspection. MR Number: 2-95-0057 Robinson 2 Date: 07/03/95 Hartsville,South Carolina Dockets: 50-261 PWR/W-3-LP Subject: RPS ACTUATION Reportable Event Number: 29009 Discussion: On June 30, 1995, the licensee reported a reactor trip due to a spurious closure of the B Main Steam Isolation Valve. Subsequent licensee transient evaluation revealed that the clip securing the fuse which supplies 125 VDC to one of two solenoid operated instrument air valves which in turn supply open motive force to the B MSIV was very loose. The licensee concluded that when an SRO replaced a fuse adjacent to the fuse for the B MSIV, the vibration/movement resulted in the fuse for the B MSIV losing electrical continuity. This in turn caused the solenoid operated instrument air valve to de-energize which caused the MSIV to close. The licensee replaced the faulty fuse clip as well as the fuse clip for one of the two solenoid operated instrument air valves for the C MSIV.
