MR Number: 1-95-0069 Calvert Cliffs 2 Date: 05/25/95 Lusby,Maryland SRI PC Dockets: 50-318 PWR/CE Subject: MANUAL REACTOR TRIP IN ANTICIPATION OF LOSS OF VACUUM Reportable Event Number: 28849 Discussion: At approximately 12:49 pm on May 24, 1995, while electricians were conducting a job walkdown on the water treatment motor control center (MCC 208), a loose plate fell into the MCC causing a short. This resulted in a voltage dip on non-vital electrical distribution busses. As a result of the voltage dip, five of the six circulating water pumps tripped on low voltage protection signals. A battery charger was also deenergized. Operators in the control room tripped the reactor about a minute later in anticipation of a loss of vacuum on the main condenser. The electrician at the MCC received flash burns, and was transported to a local hospital for treatment. BGE is conducting an investigation into the causes of the event including why the battery charger was deenergized. A Significant Incident Findings Team (SIFT) has been convened by BGE to review the causes and personnel actions related to the event, and develop corrective action recommendations. Reportable Event Number: N/A Discussion: On May 24, 1995, the licensee transported its first loaded TN-40 spent fuel storage cask from the plant auxiliary building to the ISFSI storage site (approximately 1/2 mile) and placed the cask on the seismic category I storage pad. Seal pressure monitoring equipment was installed and placed into service. The evolution was conducted safely and proceeded without incident. Surface temperature of the cask was approximately 130 degrees Fahrenheit and contact dose rates were less than about 5 mrem gamma and neutron.
