BERNHARD (404)331-4664 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV MAY 24, 1995 Licensee/Facility: Notification: Washington Public Power Supply System MR Number: 4-95-0075 Washington Nuclear 2 Date: 05/24/95 Richland,Washington Resident Inspector Dockets: 50-397 BWR/GE-5 Subject: MAINTENANCE-RELATED DAMAGE DURING OUTAGE Reportable Event Number: N/A Discussion: On May 23, 1995, at approximately 5 p.m. (PDT), with the plant in Mode 4 in Refueling Outage R10, during testing of the high pressure core spray diesel generator (HPCS DG), the licensee identified that smoke was emanating from the potential transformer windings of the HPCS DG voltage regulator. The retest followed maintenance that included replacement of a motor-operated potentiometer in the HPCS DG voltage regulator circuitry. The licensee's preliminary assessment of the failure of the potential transformer was that the HPCS DG had been operated during the retest for an extended period of time at less than normal engine speed. The licensee is investigating the extent of the damage to the regulator and will perform a root cause evaluation. This issue has the potential to impact the scheduled May 31, 1995 restart from Refueling Outage R10. The licensee also experienced maintenance-related problems involving the incorrect installation of 24 VDC Agastat relays in the 120 VAC standby liquid control system circuitry and the inadvertent dropping of a turbine building crane brush rigging into the high pressure turbine, which was open for maintenance. The licensee is evaluating the turbine for damage and investigating the root cause of these errors. The resident inspectors responded to the licensee's announcement of smoke in the HPCS DG room and observed the licensee's response.
