Date: 05/18/95 One Research Drive,Connecticut Dockets: 03032527 License No: 06-28699-01 Subject: Potential Tritium Contamination Reportable Event Number: N/A Discussion: On April 27, 1995, during an NRC inspection at the Forschner Group, Inc. facility in Shelton, Connecticut, of activities authorized by their license, the inspector noted that assembly/disassembly and repairs of tritium painted watch subcomponents were being conducted at the facility. Such activities neither were reviewed nor authorized by the NRC. On May 4, 1995, the NRC, Region I office, issued a Confirmatory Action Letter (CAL), No. 1-95-007 to Forschner Group, Inc. to stop immediately all assembly/disassembly and repair activities until surveys of personnel involved and the work area has been performed by the licensee to evaluate the extent and magnitude of potential tritium contamination. On May 5, 1995, Forschner Group, Inc. informed the Region I office that a black light survey was performed by their consultant, NES, of the work area in question, the hallway adjacent to the work area, and on personnel. They stated that luminous painted watch subcomponents were found at the work station, and inside of a vacuum cleaner bag for the work area. No luminous painted subcomponents in other areas or on the personnel involved were detected. The licensee's consultant performed contamination surveys of areas and personnel and conducted bioassays on the four workers involved in watch repair. Area surveys indicated that the repair facility was contaminated with low levels of removable tritium with a maximum of 117,000 dpm/100 centimeter squared. No tritium contamination was found outside the watch repair room with the exception of a small area of contamination in the men's rest room. The bioassays indicated minimal intake of tritium by workers with total committed dose of less than 10 millirem. On May 17, 1995, the licensee informed Region I that decontamination of all affected areas has been completed. A decontamination report will be submitted to Region I with a request to release the areas for unrestricted use. Grand Gulf 1 Port Gibson,Mississippi Dockets: 50-416 BWR/GE-6 Subject: SEVERE WEATHER STORM AT GRAND GULF Reportable Event Number: 28827 28830 Discussion: At approximately 4:15 p.m., on May 18, 1995, CST, a severe hail storm along with high winds struck the Grand Gulf Nuclear Station. The storm lasted approximately 40 minutes. There were no personnel injuries and no safety related equipment was affected. All sources of offsite power remained available during the event. All three roads allowing egress to the site were temporarily impassable due to downed trees. The roads were cleared within two hours after the storm. Power was lost across the Mississippi river in Tensas Parish, Louisiana, disabling all emergency sirens. Alternate methods of notification were available. The licensee notified the NRC Operation Center of the loss of a portion of their sirens. In addition, a courtesy notification was made to inform the NRC of damage to security barriers which have since been repaired. Maximum wind velocities of 69 MPH were recorded at the meteorological tower. Damage to temporary structures, automobiles and trees was sustained. Some minor damage to permanent administrative buildings occurred. An NRC inspector was onsite during the entire event and recovery. By about 9:00 p.m., CDT, approximately 79% of the offsite sirens had been returned to service. The licensee is working to return the remaining failed sirens to service. Regional Action: A courtesy notification regarding the Grand Gulf offsite siren system was made by NRC Region II to the FEMA Region IV office at about 7:00 p.m., EDT, on Thursday, May 18, 1995, and an update was provide at 9:30 a.m., Friday, May 19, 1995.
