The main generator was disconnected from the grid at 2:30 p.m., and the reactor trip breakers were opened at 4:10 p.m. The reactor shutdown began a few hours earlier than originally planned due to a leak that developed in a the cooling water system for a containment fan coil unit. In addition to refueling, major work will include: steam generator eddy current inspection and tube repair, inspection of service water system pipe coatings, 4160 volt safeguards bus maintenance, containment fan coil unit valve replacement and cooling coil replacement, motor valve testing, and reconstitution of a leaking fuel assembly. KROPP (708)829-9663 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV MAY 15, 1995 Licensee/Facility: Notification: Omaha Public Power District MR Number: 4-95-0069 Ft Calhoun 1 Date: 05/15/95 Fort Calhoun,Nebraska SRI Telecon Dockets: 50-285 PWR/CE Subject: REACTOR STARTUP FOLLOWING REACTOR COOLANT PUMP REPAIRS Reportable Event Number: N/A Discussion: The licensee initiated a plant startup at 1:40 a.m. on May 15, 1995. Criticality was reached at 3:13 a.m. The startup ends a 4-day shutdown initiated on May 11 at 11:05 a.m. (CDT) after confirmation of component cooling water leakage into the Reactor Coolant Pump (RCP) D bearing lube oil through the heat exchanger. The licensee manually tripped the reactor from 100 percent power to prevent damage to the RCP motor. The plant was stabilized in Mode 3 (Hot Shutdown) while inspection and repairs were made. While in Mode 3, the licensee performed eddy current testing on all four RCP's bearing oil coolers and decided to plug those tubes that showed greater than 40 percent wall thinning. Of a total of 72 tubes in the RCP D heat exchanger, 6 tubes required plugging. The licensee's design engineering position on tube plugging was that a maximum of 8 tubes could be plugged in each heat exchanger. The inspections on the remaining heat exchangers resulted in one tube plugged on RCP C, zero tubes plugged on RCP B, and four tubes plugged on RCP A. One of the tubes plugged on RCP A was done during the recent refueling outage. The licensee's present plans are to replace all four heat exchangers during the next refueling outage which is scheduled to start September 1996. Regional Action: For information only. Contact: R. E. MR Number: 4-95-0070 Arkansas Nuclear 1 Date: 05/15/95 Russelville,Arkansas Licensee Telecon Dockets: 50-313 PWR/B&W-L-LP Subject: GRANTING OF ENFORCEMENT DISCRETION Reportable Event Number: N/A Discussion: On May 12, 1995, NRR granted enforcement discretion to Arkansas Nuclear One, Unit 1, from the requirements of Technical Specification 4.5.2.1.2(a)(1), which required that service water flow to each reactor building emergency cooling train be verified at greater than or equal to 1200 gpm. The enforcement discretion is granted until an exigent Technical Specification can be processed. The licensee experienced a failure of a fan motor in one of two chillers in a single train of reactor building emergency cooling. The licensee performed an engineering evaluation to demonstrate that a single chiller, with flow less than 1200 gpm, in the affected train has sufficient heat removal capability under design basis accident conditions. The licensee established administrative controls to implement the results of the evaluation and expects the fan motor to be rewound and returned to service in approximately 5 weeks. Regional Action: The Region will review the engineering evaluation and ensure that compensatory actions specified in the evaluation are implemented. Contact: T. Reis (817)860-8185 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV MAY 15, 1995 Licensee/Facility: Notification: Washington Public Power Supply System MR Number: 4-95-0071 Washington Nuclear 2 Date: 05/15/95 Richland,Washington Licensee to SRI Dockets: 50-397 BWR/GE-5 Subject: OVERTURNED DRUM OF HIGHLY RADIOACTIVE DECONTAMINATION FILTERS Reportable Event Number: N/A Discussion: With the facility in Refueling Outage R10 and Mode 5 (Refueling) at 5 a.m. (PDT), on May 15, 1995, while transporting highly contaminated filters (up to 20 R/hr on contact) originating from the chemical decontamination of the reactor water cleanup system, an open 55-gallon shielded drum containing the filters overturned when laborers were passing through a doorway in the reactor building. The laborers immediately left the area and notified health physics. The licensee immediately restricted access to that area of the facility. Because the filters were double-wrapped in polyethylene bags, this event was only a potential exposure concern and not a surface or airborne contamination problem. There was no release of radioactivity. No contamination spilled from the wrapped filters. At approximately 10:30 a.m. after extensive planning, licensee laborers and health physics technicians uprighted the 55-gallon drum and its contents. A total of 471 mrem exposure was absorbed during the uprighting evolution. The licensee then transported the drum to its shielded storage area. The licensee formed an incident review board (IRB) to determine the cause of the event and establish corrective actions to prevent event recurrence. Regional Action: The licensee notified the senior resident inspector of the event at approximately 5:45 a.m. and he responded to the site. The inspector observed the planning sessions and the mockup uprighting evolutions.
