MR Number: 4-95-0065 River Bend 1 Date: 05/11/95 St Francisville,Louisiana Senior Resident Inspector Dockets: 50-458 BWR/GE-6 Subject: PRIVATE PLANE EMERGENCY LANDED WITHIN THE OWNER CONTROLLED AREA Reportable Event Number: 28784 Discussion: On May 10, 1995, at approximately 3:52 p.m. (CDT), a small, private airplane (4-person, single engine) emergency landed approximately 1.5 miles from the protected area within the owner controlled area. The pilot landed the airplane on the West Feliciana Parish river access road that leads to the River Bend makeup water intake structure. The licensee reported that the engine lost power and forced the pilot to land the aircraft. The pilot received no injuries, and no damage to the aircraft occurred. The individual had left Memphis, TN headed for Lafayette, LA when the plane began losing power. The plane diverted to the New Roads airport across the river from St. Francisville, which is approximately 2 miles from the landing site. The licensee's security force promptly responded to the landing after plant employee's reported the landing. The licensee contacted the state police and the West Feliciana Parish sheriff who promptly responded to the scene and determined that the airplane carried no contraband. The licensee verified that they did not meet the emergency action levels for declaring an unusual event but did make a 4-hour nonemergency information call to the NRC Operations Officer. The licensee cordoned off the plane until a plan was developed for its removal. Regional Action: The resident inspectors are monitoring the licensee activities during their routine duties. MR Number: 4-95-0066 Arkansas Nuclear 1 2 Date: 05/11/95 Russelville,Arkansas Resident Inspector Dockets: 50-313,50-368 PWR/B&W-L-LP,PWR/CE Subject: SMALL ENGINE PLANE CRASH AFFECTS PLANT OUTPUT Reportable Event Number: N/A Discussion: A small craft airplane crashed south of Little Rock (approx. 90 miles from site) during the morning of May 10, 1995 and affected electrical output at the ANO Units 1 & 2 facility. Breakers to the 500kV Mayflower line had to open to affect repairs on that line. Dispatchers requested that total site output be reduced to 1100 MWe. Accordingly, Unit 1 was reduced to 57 percent power and Unit 2 to 72 percent. Repairs are expected to be completed today and the units will return to full power operation. Each of the units still has at least two independent sources of offsite power. Regional Action: Routine resident inspector coverage is planned. Contact: T. MR Number: 4-95-0067 Palo Verde 2 Date: 05/11/95 Wintersburg,Arizona PHONE CALL FROM RESIDENT INSPECTORS Dockets: 50-529 PWR/CE80 Subject: POWER DECREASE DUE TO HIGH STEAM GENERATOR CHEMISTRY CONDITIONS Reportable Event Number: N/A Discussion: At approximately 10:45 a.m. on May 10, 1995, with Palo Verde Unit 2 at 100 percent power, operators observed a sharp increase in sodium and sulfate concentrations in the 1A condenser hotwell and a corresponding increase in steam generator sodium and sulfate concentrations. Within minutes, steam generator sodium and sulfate concentrations increased from less than 5 parts per billion to over 300 parts per billion. Unit 2 operators entered the procedure for a condenser tube rupture and began reducing power to 40 percent. At approximately 12:30 p.m. on May 10, Unit 2 was stabilized at 40 percent power, and the 1A condenser hotwell was isolated. The licensee drained the hotwell, performed a leak test, and did not identify any leaking circulating water tubes. The licensee inspected other possible sources of condenser in-leakage and suspected that the source was the auxiliary steam condenser tank which returns to the 1A condenser hotwell. The licensee secured the auxiliary steam condenser tank to the hotwell and was able to reduce the sodium and sulfate levels in both steam generators. At 6:00 a.m. on May 11, the licensee increased Unit 2 power to 48 percent and was holding at this power while restoring steam generator chemistry to normal levels. The licensee was investigating the exact source of the impurities in the auxiliary steam system. Palo Verde Unit 1 is in Mode 5 during a refueling outage, and Unit 3 is operating at 100 percent power. Regional Action: The resident inspector responded to the control room and observed portions of the Unit 2 downpower.
