This notice alerts addressees to a recent incident in which the patient couch of a Leksell Gamma System Model 23016 ("gamma knife") unit failed to retract from the treatment position because of a failure of a two-position, solenoid-operated valve on the hydraulic system of the unit. Holahan, NMSS (301) 415-7847 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION I MAY 10, 1995 Licensee/Facility: Notification: Non-Nrc Licensee MR Number: 1-95-0067 Salvage Yard Date: 05/09/95 Belmont,Massachusetts License No: N/A Subject: RADIOACTIVE MATERIAL IN A CYLINDER FOUND AT SALVAGE YARD Reportable Event Number: N/A Discussion: On May 9, 1995, at 5:00 p.m., a call from an individual member of the public was received by the Headquarters Duty Officer and dispatched to Region I. The owner of a salvage operation in Belmont, Massachusetts notified Region I of an object with a radioactive material label marked Danger Cesium-137. The object was a cylinder of approximately one-half inch in diameter and 2-3 inches in length. The salvage yard operator described the object as a glass cylinder with a metal ring at each end and two small ball-bearing like spheres inside the cylinder a quarter of an inch apart. The individual informed Region I that he was unable to ascertain when he received the cylinder or where it came from. The individual stated that he inserted the cylinder into a metal pipe and placed it under a car at the salvage yard. Region I instructed the individual to keep at a distance from the object and secure access to the salvage yard. The Commonwealth of Massachusetts and the U. S. Enfironmental Protection Agency were immediately notified for identification and proper control and disposal of the object. The Massachusetts Department of Radiation Protection dispatched response personnel to the salvage yard. Region I maintained contact with the individual until the State of Massachusetts response personnel arrived at the salvage yard. Upon arrival of the Massachusetts response personnel at the salvage yard the area was contolled, radiological surveys were performed, and the object was retreived. The radiological surveys indicated that the object contained trace amounts of Cesium-137. Contact dose rates of 0.1mR/hr gamma and 0.3 mR/hr betta-gamma were measured. The object posed negligible radiation exposure hazard due to the very low level of Cesium-137. The object was taken by the response personnel for proper disposal. Contact: Mohamed M. Shanbaky (610)337-5209 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV MAY 10, 1995 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-95-0064 Cooper 1 Date: 05/10/95 Brownville,Nebraska Senior Resident Inspector Dockets: 50-298 BWR/GE-4 Subject: ENTRY INTO EMERGENCY PROCEDURES DUE TO HIGH RIVER LEVEL Reportable Event Number: N/A Discussion: The licensee has entered their emergency plan procedure for high Missouri river level. The river level is currently at 891 feet 8 inches and rising. The licensee's procedures require increased monitoring of river level at 891 feet. The emergency plan requires the declaration of a Notification of Unusual Event at 899 feet and the Technical Specifications require a plant shutdown at 902 feet at which time an Alert is declared. In July 1993 the Missouri River crested at approximately 900 feet and the licensee initiated a plant shutdown as a precautionary measure at 897 feet 11 inches because of flooding of evacuation routes within the 10 mile emergency preparedness zone. A Notification of Unusual Event was declared on July 23, 1993. At that time Region IV dispatched a team to the site while the Region entered the Monitoring Mode. Currently, the Army Corps of Engineers predicts the river to crest at 895 feet in the absence of more rain in the northern Missouri river basin.
