The NRC is issuing this generic letter to inform addressees of a new staff position on the use of Nuclear Management and Resources Council/Electrical Power Research Institute (NUMARC/EPRI) Report TR-102348, "Guideline on Licensing Digital Upgrades," dated December 1993, as acceptable guidance for determining when an analog-to-digital replacement can be performed without prior NRC staff approval under the requirements of Section 50.59 of Title 10 of the Code of Federal Regulations (10 CFR 59.59). Loeser, NRR (301) 415-2825 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION III APRIL 25, 1995 Licensee/Facility: Notification: U.S. Enrichment Corporation MR Number: 3-95-0072 Paducah Gaseous Diffusion Plant Date: 04/25/95 Paducah,Kentucky PHONE CALL FROM RESIDENT INSPECTOR Dockets: 07007001 Subject: SUSPICIOUS VEHICLE Reportable Event Number: N/A Discussion: On April 24, 1995, at about 12:20 a.m. (CST), a Paducah Gaseous Diffusion Plant security officer, while on patrol of the plant access road, observed a vehicle (pick-up truck) parked along side the road with its parking lights on. The officer approached the vehicle and saw a 60-70 year old white male talking on a cellular phone. When the officer identified himself, the individual became very belligerent and began using profanity towards the officer. The officer noticed a handgun on the front seat near the individual and a shotgun leaning against the seat. The officer called for back-up and instructed the individual not to touch the weapons and to keep his hands in sight. The individual placed his vehicle in gear and drove off disregarding a stop sign at the intersection. Local law enforcement was notified and responded. The McCracken County Sheriff department believes that they know this individual. The incident is being investigated by local law enforcement and the FBI. No charges have been filed against the individual. The local media was notified. The plant has been on a heightened awareness status since the Oklahoma City bombing and the bomb threats at the Portsmouth Gaseous Diffusion Plant. The gaseous diffusion plant is under the jurisdiction of the Department of Energy. The NRC has resident inspectors at the Paducah facility in preparation for the NRC assuming regulatory jurisdiction over the site in October 1995. MR Number: 3-95-0073 Kewaunee 1 Date: 04/25/95 Kewaunee,Wisconsin SRI VIA PC Dockets: 50-305 PWR/W-2-LP Subject: SAFETY INJECTION PUMP DAMAGED DURING FULL FLOW TESTING Reportable Event Number: N/A Discussion: On April 20, 1995, during performance of safety injection system flow testing while shutdown in a refueling outage, the flow path for the B safety injection pump to the reactor vessel became obstructed by metallic debris clogging the throttle valves. A flow path to the reactor coolant loops was demonstrated to have remained available for all other sources of injection. The source of the debris was discovered to have been the B safety injection pump, which had become severely damaged during the test. Investigation revealed that the pump's thrust bearing had failed during the surveillance test causing the impeller to contact the pump casing. The licensee's investigation of the root cause for the damage is continuing. The licensee is planning to unload the core, core barrel, and vessel internals and perform a systematic inspection for any additional debris.
