MR Number: 2-95-0042 North Anna 2 Date: 04/11/95 Richmond,Virginia Dockets: 50-339 PWR/W-3-LP Subject: EROSION/CORROSION IN STEAM GENERATOR FEEDRINGS Reportable Event Number: N/A Discussion: During the present steam generator replacement outage portions of the original steam generator internals, including the feedrings, are planned to be reused. Boroscopic inspections inside the feedrings identified areas of wall thinning adjacent to the J tubes. Visual inspection of the C steam generator revealed a three quarter inch hole in the top of the feedring next to J tube Number 4. The wall thinning appears to be similar to that discovered last year at Diablo Canyon 1. The licensee determined that the most likely mechanism is a combination of corrosion initiated at the crevice formed where the J tubes extended into the feedring and erosion due to eddy flows around the J tube stubs inside the feedring. The feedring in C steam generator has been affected more than other two steam generators' feedrings. Presently, the licensee has determined that a feedring section containing 13 J tubes requires replacement in the C steam generator. Weld buildup will be used to repair the affected areas in the B steam generator. No repairs have been deemed necessary for effected areas in the A steam generator. MR Number: 2-95-0043 Brunswick 1 Date: 04/11/95 Southport,North Carolina Dockets: 50-325 BWR/GE-4 Subject: LOSS OF SHUTDOWN COOLING - UNIT 1 Reportable Event Number: 28659 Discussion: At 1:58 p.m., on April 10, 1995, Brunswick Unit 1 experienced a loss of shutdown cooling. At the time, a modification was in progress to remove a steam line tunnel radiation monitor. The de-termination of the relay neutral wire caused activation of a low level 1 relay which resulted in a half group 2, 6, and 8 primary containment isolation. The result was the closure of the outboard shutdown cooling containment isolation valves, in addition to other components. The licensee re-landed the lead and cleared the group isolations which permitted starting of the RHR pumps at approximately 2:50 p.m. During the event, the licensee maintained spent fuel pool cooling and supplemental spent fuel pool cooling. Monitored temperatures remained effectively steady. At the time of the event, the licensee still had approximately 55 fuel bundles in the reactor vessel. The licensee's investigation has determined that the cause of the inadvertent isolation was an inadequate modification installation package. The package should have required jumpering of a connection before de-terminating the relay neutral wire. Regional Action: The resident inspectors responded to the control room. MR Number: 3-95-0054 Point Beach 1 2 Date: 04/09/95 Two Rivers,Wisconsin via LICENSEE Dockets: 50-266,50-301 PWR/W-2-LP,PWR/W-2-LP Subject: DEMONSTRATION Reportable Event Number: N/A Discussion: On April 9, 1995, a planned demonstration was held at a county park located 1.5 miles from the Point Beach Nuclear Plant to protest dry cask fuel storage. Approximately 40 people participated in the demonstration. Following demonstration activities at the park the participants marched to a licensee's site boundary owner controlled area gate (OCA) gate located approximately 3/4 of a mile from the plant. Three demonstrators crossed the OCA boundary and were arrested for trespassing by county sheriff authorities. No injuries or damage to property occurred. Effective 1 May 1995, Mr. Keith Schwartz, currently the Station Manager at Byron Station, will replace Mr. Tony Broccolo as Station Manager of Zion Nuclear Generating Station. Mr. Kurt Kofron, currently the Station Manager at Braidwood Station, will replace Mr. Schwartz as Station Manager of Byron Station. MR Number: 3-95-0056 Dresden 2 3 Date: 04/11/95 Morris,Illinois RESIDENT INSPECTOR Dockets: 50-237,50-249 BWR/GE-3,BWR/GE-3 Subject: NUMEROUS CONTAMINATED MATERIALS FOUND OUTSIDE RADIATION POSTED AREA Reportable Event Number: N/A Discussion: On April 3 the licensee identified a contaminated oil filter during a routine radiation survey of garbage in the Clean Garbage Building (CGB) which is located outside of the Radiologically Protected Area (RPA). Subsequent surveys identified 3 additional contaminated items in the CGB. In response, the licensee initiated a multi-discipline investigation team on April 5. The team's initial actions included a material lockdown of the site and a comprehensive survey of clean areas with about 40 radiation protection technicians from Dresden, Braidwood, Zion, and Byron stations. As of April 11 with about 50 percent of the site surveyed, 136 contaminated items have been identified. Most items had fixed contamination at levels less than 5,000 disintegrations per minute per frisk. The highest level of contamination identified on an item was 140,000 disintegrations per minute per frisk. Part of the material lockdown included placing areas not yet surveyed in "quarantine." Disciplinary action was taken against ten individuals for failing to follow the material lockdown requirements. Planned licensee actions include revisions to administrative controls, review of past offsite unconditional releases, improved physical controls for RPAs, and site cleanup. MR Number: 3-95-0057 Braidwood 1 Date: 04/11/95 Braceville,Illinois SRI PC Dockets: 50-456 PWR/W-4-LP Subject: UPDATE ON REACTOR TRIP DUE TO FAILED INSTRUMENT BUS INVERTER Reportable Event Number: 28652 Discussion: On April 9, 1995, at 3:11 am (CST), Unit 1 received a reactor trip due to high steam generator level. The apparent cause was a failure of a capacitor in the inverter firing card for Instrument Bus 111. Approximately 21 instrument cards in various loops on components on Bus 111 were also damaged due to the voltage spike generated by the failed capacitor. All class 1E related instrument loops were returned to service on April 10. Two remaining non-class 1E instrument loops will be repaired on April 11 prior to restart. The licensee had previously experienced similar problems with failing capacitors on Unit 2. During the last Unit 2 refueling outage the inverter firing cards were modified to eliminate voltage spikes that could damage instrument circuit cards. On Unit 1 the modificiation was completed on 3 of the 4 inverters on April 10. Repairs to the last inverter will be done on April 11. Restart is expected late on April 11 with the unit returning to service on April 12. Region Action: The residents and regional inspectors continue to monitor restart and maintenance activities. MR Number: 3-95-0058 Prairie Island 1 2 Date: 04/11/95 Welch,Minnesota RI PC Dockets: 50-282,50-306 PWR/W-2-LP,PWR/W-2-LP Subject: PRAIRIE ISLAND DRY CASK DRY RUN Reportable Event Number: N/A Discussion: On April 11, 1995, the licensee intends to perform dry run (practice) activities in the spent fuel pool with its first TN-40 spent fuel storage cask. Activities performed will include: placing the TN-40 cask into the spent fuel pool, loading a dummy fuel assembly into the cask and removing it, placing the cask lid onto the cask while underwater, and moving the cask to the decontamination area for cleanup. Regional Action: The Region III Senior Dry Cask Storage Oversight Manager, the resident inspectors, and a regional radiation protection inspector will be onsite monitoring licensee activities.
