MR Number: 1-95-0051 Indian Point 2 Date: 04/05/95 Buchanan,New York SRI Dockets: 50-247 PWR/W-4-LP Subject: STEAM GENERATOR TUBE EXAMINATION FAILURE Reportable Event Number: N/A Discussion: During Con Edison's steam generator tube examination program, defects were identified in the No. 23 steam generator roll transition region. Con Edison is using a bobbin and CECCO 5 probe with a rotating pancake probe to confirm the indications. The CECCO 5 probe is sensitive to axial and circumferential indications. Preliminary analysis indicates that the No. 23 steam generator has 542 pluggable indications on the hot leg in the roll transition region of which 20 have some circumferential indication. Because of the quantity of indications, if plugging is required, Con Edison may be over the 25% maximum plugging limit. The FSAR describes that the basis for the 25% steam generator tube plugging level is the small break LOCA analysis. The other steam generators have similar indications in the roll transition region but not as many. The No. 21 SG has 13 indications and the No. 22 SG has 2 indications. Inspection of the No. 24 steam generator is in progress. Con Edison was recently issued a technical specification amendment to allow the repair of steam generator tubes via the implementation of an F* criteria. The F* distance is the distance of the expanded portion of a tube which provides a sufficient length of undegraded tube expansion to resist pullout of the tube from the tubesheet. This would allow tubes that are degraded in a location not affecting structural integrity of the tube to remain in service as an alternative to removal from service through the use of tube plugs. However, Con Edison's attempt to reroll several tubes has been unsuccessful because of crud buildup in the crevice. Con Edison has also submitted a technical specification change request to allow sleeving as an alternative to reroll. The review of this TS change request is in progress.
