MR Number: 1-95-0048 Indian Point 2 Date: 03/29/95 Buchanan,New York RI/NOGGLE Dockets: 50-247 PWR/W-4-LP Subject: SHIPMENT OF LOW LEVEL WASTE RECEIVED ABOVE RADIATION LIMITS Reportable Event Number: 28604 Discussion: On March 27, 1995 a shipment of low level radioactive waste, consisting of bags of Dry Active Waste (DAW) contained in two 20-foot Sealand containers, left the Indian Point Unit 2 Station for Scientific Ecology Group (SEG) in Oak Ridge, Tennessee for waste processing. The shipment arrived at SEG on March 28, 1995 and was surveyed at 1300 hours. The survey indicated at one location of the package, a contact reading of 375 mrem/hr and a two meter reading of 13 mrem/hr. At another location of the package, a reading of 11 mrem/hr at 2 meters was observed. The tractor cab reading was measured at 2.5 mrem/hr. The Department of Transportation limits are 200 mrem/hr on contact, 10 mrem/hr at 2 meters, and 2 mrem/hr in the cab. The licensee was notified by SEG at 1600 hours on March 28, 1995 and Region I was notified at 1700 hours. The licensee provided the survey results of the shipment that were made at its departure from Indian Point Station which indicated a maximum of 115 mrem/hr on contact, 8 mrem/hr at 2 meters and 1.5 mrem/hr in the cab. The licensee asked SEG to secure the trailer and has dispatched two qualified persons to conduct surveys. They are expected at SEG about noon on March 29, 1995. MR Number: 2-95-0037 Summer 1 Date: 03/29/95 Jenkinsville,,South Carolina Licensee Vice President - NucOps Dockets: 50-395 PWR/W-3-LP Subject: INJURED NRC WORKER Reportable Event Number: N/A Discussion: At approximately 9:00 a.m. on March 29, 1995, a Region II inspector conducting a routine inspection at the V. C. Summer Plant, located near Columbia, South Carolina, fell approximately 6-8 feet from a ladder on to a concrete floor in the Auxiliary Building. An accompanying inspector summoned assistance from licensee emergency personnel and the injured inspector was removed from the radiological controlled area on a backboard and in a neck brace. Neither inspector was contaminated. Shortly after the incident, the uninjured inspector complained of dizziness and shortness of breath. He was administered oxygen and he and the injured inspector were transported via ambulance to Richland County Memorial Hospital in Columbia, South Carolina. Both inspectors are reported in stable condition by hospital authorities. State radiological health personnel are in attendance at the hospital and report both inspectors are alert and currently undergoing a series of medical evaluations. Regional Action: The inspectors' supervisor is enroute to the hospital to lend any necessary support and assistance. Region II is maintaining contact with hospital officials regarding this situation. MR Number: 3-95-0044 Dresden 2 Date: 03/29/95 Morris,Illinois RESIDENT INSPECTOR Dockets: 50-237 BWR/GE-3 Subject: EXTENDED UNIT 2 FORCED OUTAGE - UPDATE Reportable Event Number: N/A Discussion: On March 28 the licensee administratively disqualified the station's nuclear engineers from performing nuclear engineering functions for Unit 2. In addition, nuclear engineering work for the operating unit (Unit 3) will require a second nuclear engineer to verify any proposed actions. This action was taken in response to a March 22 event where the technical specification fuel design limiting ratio for centerline melt (FDLRC) was unexpectedly exceeded. (See morning report of 3/28/95.) The licensee placed a hold on Unit 2 startup activities until the nuclear engineers complete remedial training and are interviewed by engineering management.
