MR Number: 3-95-0034 Braidwood 1 Date: 03/14/95 Braceville,Illinois SRI PC Dockets: 50-456 PWR/W-4-LP Subject: UNIT 1 STARTUP FOLLOWING MID-CYCLE MAINTENANCE OUTAGE Reportable Event Number: N/A Discussion: On March 13, 1995, at 2:16 (CST), Unit 1 went critical following the completion of a 20 day scheduled mid-cycle maintenance outage for steam generator tube inspections. As a result of the inspections, about 850 steam generator tubes were plugged. This was slightly lower than the number predicted by the licensee for these Westinghouse D-4 steam generators which are experiencing unexpected premature degradation. No other significant maintenance occurred during the outage. Regional Action: The resident inspectors observed portions of the startup and will continue to monitor Unit 1 as power level increases. MR Number: 4-95-0032 Comanche Peak 1 Date: 03/13/95 Glen Rose,Texas Phone Call From Resident Inspector Dockets: 50-445 PWR/W-4-LP Subject: REFUELING MAST CAMERA FELL INTO REACTOR VESSEL Reportable Event Number: N/A Discussion: On March 13, during Unit 1 core offload, fuel handlers observed an object resting on the upper internals seating area in the reactor vessel. A more detailed observation of the object determined that it was the camera lens assembly that was normally attached to the refueling mast. The lens assembly is approximately 18 inches long and 3 inches in diameter. Fuel movement was halted until the licensee evaluated the camera for missing parts and the potential for fuel damage. Using a spare camera assembly from the warehouse, the licensee concluded that the lens and light portion of the camera assembly had separated from the rotator assembly joint. The lens portion of the camera was secured to its rotator mount by three set screws on the lens section of the camera. If the set screws became loosened, the lens portion of the camera can separate from the rotator mount and would then be supported only by several small wires which apparently were insufficient to support the lens. The licensee believed they accounted for all camera parts and retrieved the lens following the completion of core offload on March 14. MR Number: 4-95-0033 Arkansas Nuclear 1 Date: 03/14/95 Russelville,Arkansas Resident Inspector Dockets: 50-313 PWR/B&W-L-LP Subject: REACTOR BUILDING EMERGENCY COOLERS NOT ANCHORED Reportable Event Number: 28521 Discussion: During refueling outage 1R11, completed in October 1993, licensee engineers discovered that the four reactor building emergency coolers were not bolted to the structural steel which provided support. The lack of bolts was discovered as part of the licensee's walkdown efforts performed for resolution of Unresolved Safety Issue A-46 (SQUG). The licensee believed that the cooling units were welded to the structural steel supports, but did not adequately verify this assumption. On March 13, 1995 licensee engineers walked down the reactor building cooling units to verify the welding details and determined that the reactor building cooling units were not welded to the structural steel. This condition has apparently existed from initial operation. Technical Specifications require that the reactor building cooling system be operable whenever containment integrity is established. Although the coolers were previously required to be operable, the current plant conditions do not require containment integrity. The licensee intends to weld these units to the structural steel prior to plant heatup above 200 degrees F. Regional Action: The resident inspectors will followup on this issue. Contact: K. M. Keuter as the new General Manager, Plant Operations. Mr. Keuter is scheduled to begin his duties the first week of April, 1995. He is currently the Vice President, Nuclear Readiness for the Tennessee Valley Authority and succeeds Mr. Dan Packer, who became Vice President, Regulatory and Governmental Affairs for the City of New Orleans in November 1994. Mr. Keuter has been with TVA since September 1991. Prior to then, he was with the Sacramento Municipal Utility District at Rancho Seco Nuclear Generating Station and with Portland General Electric Company at Trojan Nuclear Power Plant.
