MR Number: 2-95-0028 Turkey Point 3 Date: 03/10/95 Miami,Florida Dockets: 50-250 PWR/W-3-LP Subject: FOULING CAUSED BY AQUATIC GRASSES Reportable Event Number: 28506 Discussion: Significant rain storm activity following a dry spell dislodged a large quantity of aquatic grass in the cooling canal system on March 8, 1995. The grass entered the intake structure and created a high differential pressure across the traveling screens. The fouling affected the flow rates of the circulating water system and the component cooling water heat exchangers. The licensee has cleaned the traveling screens, component cooling water basket strainers and condenser water boxes. The resident inspectors have verified that the quantitY of grass in the intake structure is currently at a manageable level. As of 9:00 a.m. on March 10, 1995, the licensee was increasing power on Unit 3 with Unit 4 still shutdown in a maintenance outage. Dockets: 03013027 License No: 12-00722-06 Subject: LEAKING MUZZLE REFERENCE SENSOR Reportable Event Number: N/A Discussion: Licensee reported to the Operations Center that a muzzle reference sensor containing 10 Curies (370 GBq) tritium was leaking. Part of the information recorded on the Event Notification Worksheet was recorded in error. Subsequent information obtained from the licensee indicated that a package containing the muzzle reference sensor was opened in a hood. Upon opening the package, the radiation alarm meter in the hood sounded indicating a release of licensed material. The package did not appear damaged but a careful inspection revealed a glass vial containing the 10 Curies (370 GBq) tritium had been broken. Immediate contamination surveys of two individuals involved indicated they had not been contaminated. Other contamination surveys indicated activity inside the package of 52,820 dpm and 105 dpm at the hood floor in front of the monitor. Bioassay samples were taken from the two individuals with results pending. Additional contamination surveys are to be taken. The licensee is reviewing the event to determine Part 20 release concentrations. There was no press release. Regional Action: A followup 30-day written notification to the Region will be forwarded.
