MR Number: 3-95-0030 Zion 1 2 Date: 03/07/95 Zion,Illinois PLANT ALARA COORDINATOR VIA PHONE Dockets: 50-295,50-304 PWR/W-4-LP,PWR/W-4-LP Subject: TWO LABORERS RECEIVED INTERNAL UPTAKES OF RADIOACTIVE MATERIAL Reportable Event Number: N/A Discussion: On March 6, 1995, at about 10:45 a.m. (CST), two laborers received internal uptakes of radioactive material. The laborers had been working in the radioactive waste building loading dried mop heads into a radioactive waste storage liner. The area that the laborers were working in was posted as a high radiation area but not as a contamination zone. Following the workers exit from the work area, the workers were discovered to be contaminated. They were externally decontaminated and whole body counted. Uptakes of 1491 nanocuries and 381 nanocuries of activation and fission products were identified. Twenty hours later, whole body counts were again performed and the uptakes had fallen to 270 nanocuries and 70 nanocuries respectively as a result of biological elimination. Dose estimates, as a result of the uptakes, were 116 mrem and 30 mrem respectively. Following the incident, smears, taken in the area, indicated 30 mrem/hr smearable on the floor beneath the mop head drying racks and 100,000 cpm smearable around the top of the liner. No other elevated contamination levels were found. The 30 mrem/hr smear was analyzed and indicated the presence of both activation and fission products. The licensee had not determine where the mops heads had been used. The licensee is currently investigating this incident. Resident and regional inspectors are following licensee actions. Regional Action: Resident and regional inspectors along with regional management will monitor the licensee's investigation and corrective actions.
