Poslusny, NRR (301) 415-1402 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION II MARCH 6, 1995 Licensee/Facility: Notification: Department Of The Army MR Number: 2-95-0025 Corps Of Engineers Date: 03/06/95 Vicksburg,Mississippi Dockets: 03005062 License No: 23-01544-10 Subject: Improper Disposal of Gas Chromatograph Reportable Event Number: N/A Discussion: On March 3, 1995 the licensee notified Region II that a surplussed gas chromatograph (GC) containing an approximately 15 millicurie Nickel-63 foil source had been sent to Keesler Air Force Base, Mississippi, for disposal in September 1993. The dose rate from source in the GC is minimal. The GC had been inventoried last in January 1993. The licensee identified that the source was missing during a Radiation Protection Officer's review of the location of the GC. Keesler Air Force Base was unaware that the GC contained radioactive material. The machine was sold to a scrap broker, Southern Scrap Metals Company. The licensee has been in contact with the scrap broker and was informed that at this late date it is not possible to determine where the material is at this time. The State of Mississippi has been notified. Regional Action: A Region II inspector will followup on this event during the next routine inspection. Contact: L. This commenced the units' 13th refueling outage, which will include core shroud inspection and repair, reactor water cleanup piping replacement, torus reconditioning, HPCI turbine repairs, station blackout diesel modification, and RHR system valve and motor vibration related repairs. MR Number: 3-95-0027 Zion 2 Date: 03/06/95 Zion,Illinois SRI VIA PC Dockets: 50-304 PWR/W-4-LP Subject: INADVERTENT LOSS OF LEVEL Reportable Event Number: N/A Discussion: Unit 2 is currently shutdown for a refueling outage. On March 3, 1995, while clearing an out-of-service on 2 MOV-SI8807B; (cross-over between charging pump and safety injection pump suction valve), a flow path was established from the reactor coolant system (RCS) to the refueling water storage tank (RWST). Approximately 500 gallons of water was lost from the RCS inventory to the RWST resulting in reactor vessel level lowering approximately 6 inches. Ten feet of water covered the fuel at all times. Preliminary investigation indicates the cause of the event to be associated with configuration control following the completion of VOTES testing on March 2. The licensee's root cause investigation is continuing with the inspectors following these efforts. The refueling outage commenced on March 4 at 12:01 a.m. (CST) when the main electrical generator output breaker was opened. The licensee will replace two low pressure turbine rotors and perform a containment integrated leak rate test and steam generator tube inspections during the scheduled 45-day outage.
