MR Number: 2-95-0024 Catawba 2 Date: 03/03/95 York,South Carolina Dockets: 50-414 PWR/W-4-LP Subject: CATAWBA UNIT 2 REACTOR TRIP Reportable Event Number: N/A Discussion: This is an update to the Catawba Unit 2 Over Power delta-Temperature reactor trip which occurred on February 21, 1995, when main steam isolation valve (MSIV) B unexpectedly closed. The closure of MSIV B was attributed to a degraded digital optical isolator (DOI) in the valve's control circuit. Prior to restarting Unit 2 on March 2, 1995, the licensee tested the DOIs in the control circuits of the other three MSIVs, as well as other DOIs in similar applications. As a result, four DOIs (E-Max model #175C156) were replaced in the other three MSIV control circuits. Additionally, the licensee extended the unplanned outage to complete the following work activities: leak repairs on the underground condenser cooling water supply piping to the 2C cooling tower; replacement of the 2A residual heat removal pump casing flange gasket to correct minor leakage; and leak sealant-type repairs on several steam generator secondary manways. The unit is currently at 5% power and is expected to be on line later today. Regional Action: The resident inspectors continue to monitor the licensee's recovery/restart activities. Contact: R. V. Inspctor call to RIV Dockets: 50-528,50-529,50-530 PWR/CE80,PWR/CE80,PWR/CE80 Subject: UPDATE OF DISCOVERY OF STARTUP STRAINERS Reportable Event Number: N/A Discussion: On March 2, the licensee found startup strainers in both condensate transfer (CT) pump suction lines in Unit 1. The morning report of 3/2/95 incorrectly stated that the licensee had completed the inspections of all Priority 1 (safety significant) systems for any additional start up strainers on February 28. The CT pump suction line spool pieces had metal tags that indicated there were strainers in the system. The licensee performed ultrasonic testing (UT) that confirmed there was a strainer in the suction of the both CT pumps in Unit 1. Walkdowns and UT was performed of the CT pumps' suction lines in both Units 2 and 3 and strainers were not found. The CT pumps are used as a backup source of makeup water for safety-related cooling water systems and take a suction from the condensate storage tank. They are not controlled by the Technical Specifications but are surveillance tested every quarter. The licensee has completed a review of all the Priority 1 systems and a total of eight startup strainers have been found, which are: both containment spray (CS) pumps in Unit 2, both spent fuel pool cooling (PC) pumps in Units 1 and 3, and both CT pumps in Unit 1. The licensee has removed the strainers from both the CS pumps' suction lines and one of the strainers in the PC pump suction line. The other strainers were scheduled to be removed during the next 2 weeks. Regional Action: The resident inspectors are closely monitoring and independently verifying the licensee's evaluation results. A special inspection is scheduled for March 6, 1995. Contact: H.
