The discovery prompted the licensee to expand their original inspection scope to include additional bundles. The expanded inspection included all GE8 and GE10 fuel bundles which are planned for reload. The inspections have been completed and the results are as follows: TYPE NO. INSP NO. WITH DEBRIS NO. All but the 3 leaking fuel bundles, which were GE8's, will be returned to the core. The preliminary inspections of the three leakers has revealed that one failure was suspected of being debris induced, a second had debris in the bundle but failure cause remains unknown at this time. The third bundle had no debris but had two splits in one fuel pin of approximately 14" and 4" long. Root cause analysis for this failure is still in progress. NYPA is awaiting GE's evaluation of the debris found during these fuel bundle inspections, including the possible source of the debris and the potential consequences of having operated with the debris in the fuel bundles. Additional inspections of the leaking fuel bundles are planned. The resident inspectors are monitoring the fuel inspection activities. Regional Action: Resident followup. Reportable Event Number: N/A Discussion: On February 23, 1995, at 8:40 p.m. (CST), the licensee initiated a reactor shutdown in preparation for RFO-13. The generator was disconnected from the grid on February 24 at 2:59 a.m. officially beginning RFO-13. Plant startup is scheduled for April 15 with the generator on-line April 18. In addition to refueling the reactor, major planned activities included reactor vessel core shroud inspection, suppression pool and emergency core cooling systems strainer inspections, replacement of both of the reactor recirculation pumps' seal packages, overhaul of the "B" low pressure main turbine and the main generator, and overhaul of both reactor recirculation pump motor generators. Date: 02/24/95 San Onofre 2 Phone Call From Resident Inspector San Clemente,California Dockets: 50-361 PWR/CE Subject: CHAIN FOUND ON STEAM GENERATOR TUBESHEET Reportable Event Number: N/A Discussion: On February 17, 1995, with the Unit in Mode 5 for its Cycle 8 refueling outage, the licensee discovered some chain wedged on the tubesheet of steam generator E088 between the steam generator blowdown pipe and the cold leg tubes. The chain had links approximately 1.5 inches long and 0.75 inches wide, similar to chains used in the steam generator during the previous outage to support lead blankets. The licensee successfully removed the chain on February 23, 1995. The chain was found to be in two pieces, approximately 2 feet and 3 feet in length. The licensee has recently initiated eddy current inspections of tubes in this steam generator. The inspection of the tubes possibly affected by the chain is expected to be completed around March 6. The licensee is reviewing the foreign material exclusion controls in place during the previous outage to try to determine how control was lost for the pieces of chain, and to determine if other materials could have also been missed. Additionally, the licensee plans to perform a detailed inspection of the secondary side of the steam generator following the completion of the ongoing feedwater ring repairs and other secondary side work.
